Physical Therapist Therapy Coordinator Outpatient

GENERAL PURPOSESetting: Treating outpatient and home health onsite assisted and independent living communitiesLocation: All Saints, Shakopee, MNHours: Monday through Friday, full-timePerforms evaluations and develops effective patient treatment plans to restore, maintain or prevent decline of patient function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing outpatient and home health care.

Services may be performed in therapy clinic or individual patient residence.

Coordinates the program and services provided at assigned location(s), including outpatient and home health therapy and wellness.ESSENTIAL JOB DUTIESProvides initial physical therapy assessment when requested in accordance with the patient’s plan of care.

Plans and administers prescribed physical therapy treatment programs.

Meets the patient’s goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining treatment plans in consultation with physicians, and by prescription.

Helps patients accomplish treatment plan and secures necessary supplies and adaptive equipment to facilitate independenceAdministers therapy treatments according to a treatment plan approved by the attending physician.

Directs treatments given by assistants.

Complies with regulatory conditions of participation for outpatient and home health servicesEvaluates effects of therapy treatments by observing, noting and evaluating patient’s progress; providing medically necessary treatment plan and modificationsDocuments patient care services by charting in patient and department records according to accepted regulatory, corporate and professional guidelines.

Records daily treatment charges per corporate procedures to allow accurate and complete billing.

Documentation may be completed using a company furnished device.Protects patients and employees by adhering to treatment location infection control policies and protocols and keeping information confidential per company and federal requirements.

Maintains safe and clean working environment by complying with treatment location and department procedures, rules and regulations.Provides services in multiple settings (including outpatient clinics in stand-alone and host (i.e.

AL, IL) locations and patient homes as determined by supervisor and patient/resident needPartner with customer to drive the development of new business through referrals and the community.Serves as liaison to host community management, ensuring customer and patient satisfaction.Performs marketing activities such as physician sales, resident and community in-services and tours Attend customer meetings to provide care coordination at case conference and support interdisciplinary care coordination.Communicate with Director of Outpatient Services on all rehab related issues in a timely manner.

Relay communications from Directors of Outpatient Services to staff when requestedAssist Director of Outpatient Services in scheduling of staff around PTO, absences and census fluctuations that assures proper staffing levelsComplete intake paperwork including physician orders and insurance information.

Ensure billing department receives all documents necessary to complete invoicing and collection; maintain office files.QUALIFICATIONSGraduate from Physical Therapy programCurrent license as a Physical Therapist in the state of practice as requiredMinimum one (1) year experience in an outpatient care or home health settingMust meet Clinical Competency requirementsAbility to travel within 50 minutes of your assigned primary building(s) as needed.Current CPR certification, if practicing in home healthMust be capable of maintaining regular attendanceKNOWLEDGE, SKILLS, ABILITIES & BEHAVIORSAbility to communicate in English, both verbally and in writingGood problem-solving skillsGood computer skills with ability to learn company or customer systemsMust be able to maintain confidentiality regarding patient, employee and company proprietary informationMust have the ability to relate professionally and positively and work cooperatively with patients, families, caregivers, and other employees at all levelsPHYSICAL AND SENSORY REQUIREMENTS Medium work involves lifting no more than 50 pounds at a time with frequent lifting or carrying of objects weighing up to 25 pounds.
