Join the AccessCNY TeamFull Time Temporary Opening with the ability to go PermanentCovering from December 2021-June 2022Now offering a $2,500 sign-on bonusMain Function:Provide Physical Therapy services for children and adults with a range of developmental, physical and/or health-related disabilities.Duties:Clinical Services.Provide clinical Physical Therapy services for an assigned caseload of participants in conjunction with utilization requirements for the position.Specialties may include seating and positioning, assistive technology, early intervention, aquatics and sensory integration.Documentation.Write progress notes, treatment plans, annual reports, discharge summaries and assessments according to departmental and payer standards.Document services rendered for billing and quality/productivity monitoring.Complete billing form for all services rendered.Professionalism.Maintain appropriate contact with assigned participants and their families/representatives.Participate in workshops and other professional development activities.Maintain current knowledge in discipline.Assist with discipline support, orientation of new staff and consultation to other departments/agencies as needed and appropriate.Teamwork.Attend and participate in departmental, committee and/or team meetings.Serve as a member of a transdisciplinary team sharing own knowledge and skills, role releasing when appropriate.Participate in agency quality assurance, fundraising and/or strategic planning activities.Planning/Purchasing.Identify needs for therapy supplies and equipment.Assist with selection and purchasing of equipment and supplies within budget parameters and administrative protocols.Perform other duties as assigned.Scope of Responsibility:In keeping with Agency policies and procedures, professional standards and applicable regulations use own experience and judgment to carry out the Agency mission and assist with the day-to-day operation of the Comprehensive Clinical Services department.
Services must be provided in conformance with NYS and professional guidelines.

Position involves access to confidential information that must be respected.Education & Experience:B.S.
Physical Therapy required.
Masters preferred.
Licensure in New York State required.
One or more years of experience with evaluation and provision of physical therapy services to individuals with disabilities preferred.Conditions of Employment:Must demonstrate ability to work effectively with individuals with disabilities.
Must be able to lift 45 pounds.
Must complete a pre-employment physical (Post Offer) which is sufficient in scope to ensure that the employee is free from a health impairment which is of potential risk to patients or which may interfere with the performance of his/her duties as required by the New York State Department of Health (Article 28).
A Health Assessment form must be completed annually thereafter.
Must participate in database check through the State Central Registry of Child Abuse and Maltreatment.
Documentation sufficient to provide credentialing with Medicare and private insurances must be provided.
Must meet continuing competency requirements.
Must have basic word processing and email skills.
