Physical Therapist

ConcertoPACE of Los Angeles is seeking a highly motivated and energic Physical Therapist who will:(br(* Deliver physical therapy to maintain or improve participants’ baseline functional status.

Serves as an active Interdisciplinary Team (IDT) member to formulate the care plan for the PACE participants.

Participate in other IDT setting that plan, coordinate, and monitor the care of ConcertoPACE participants.

Evaluate the participants at the center and or home to ensure safety of the participants at both settings.(br(* Perform initial and annual assessments for all participants and corresponding plans of care.

Perform semi-annual reassessments and unscheduled change of condition assessments for all participants receiving physical therapy services as well as corresponding plans of care.(br(* Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans.

This includes but is not limited to active and passive exercise, muscle re-education, gait, and functional training of ADLs.

Treatments include massage, heat, water electricity.(br(* Assist in the delegation of therapy plans for specific participants according to complexity of participants’ needs and nature of required physical therapy procedures.(br(* Assist in ordering, tracking, managing, and maintenance of equipment.(br(* Collaborate with nursing home and assisted living staff regarding the rehabilitation needs for participants and refers participants to other IDT members as needed.(br(* Direct or supervise group rehabilitation activities.(br(* Solicit ideas from staff for ongoing improvement in the efficient functioning of the rehab services.(br(* Train family members, caregivers, and other staff members in methods to enhance mobility, safety and functional independence of participants.(br(* Support outreach and enrollment activities by providing informational seminars on area of expertise to prospective participants(br(* Act as a representative of ConcertoPACE in the broader community and/or clinical network.(br(* Identify defective durable medical equipment (DME) and takes appropriate action to repair or replace items promptly.(br((br(Requirements(br((br(* Graduate of a physical therapy program accredited by the American Physical Therapy Association(br(* Possess a current California Driver’s License and good driving record Must maintain current automobile insurance(br(* Working knowledge of computers and Microsoft Office applications.(br(* Compliance with all applicable federal, state, and HIPPA regulations.(br(* Must be able to lift/carry up to 25 pounds.(br((br(Job Type: Full-time(br((br(Medical Specialty:(br(* Geriatrics(br(* Home Health(br((br(License/Certification:(br(* Physical Therapy License (Preferred)(br(* CPR Certification (Preferred)(br(* Driver’s License (Required)(br((br(Work Location: One location
