Pharmacist – Per Diem, Variable (LACH)

The Pharmacist performs the day to day responsibilities of drug distribution, drug order entry, processing and verification. Performs therapeutic intervention as needed, drug information and consultation to other professional staff and patients, the maintenance of patient profiles, attendance at cardio-pulmonary emergencies, drug therapy monitoring, and drug compounding. Performs and supervises medication order entry, unit dose cassette filling, preparation of all sterile medications, narcotic dispensing and accounting, and participates in all code blues. The Pharmacist is also responsible for directing technicians and performing inventory control, as assigned by the supervisor/director.
Job Responsibilities/Duties
- Reads, interprets and verifies physician orders. Assesses medication orders initially and on an ongoing basis for appropriateness. Prepares and distributes drugs to patients and ensures optimal formulary compliance.
- Accurately compounds and dispenses extemporaneous preparations and controlled substances. Appropriately prepares parenteral products using aseptic technique. Implements appropriate age-related therapy, pharmacokinetics dosing, and performs and documents drug therapy interventions.
- Collaborates with other team members in providing correct drug and dosage form of delivery for age group and collaborates with nursing to evaluate the effectiveness of therapy considering the age of patient. Participates as an active member of the Code Blue Team.

- Actively participates in the training/education of new and existing staff. Serves as an effective participant on clinical rounds and participates in meetings and programs, as needed/assigned, to promote patient care and provide understanding and optimal drug therapy.
Qualifications
Minimum Education: Graduate of an accredited School of Pharmacy recognized by the California State Board of Pharmacy required. Pharm.D. and post-doctoral residency preferred.
Minimum Experience: Two (2) years’ experience in clinical pharmacy or a residency program required. Ability to maintain flexibility and composure in a constantly changing environment. Ability to read, write and communicate effectively in English. Strong time management skills and ability to effectively prioritize tasks. Ability to establish and maintain effective working relationships within the organization.
Req. Certification/Licensure: Licensed by the CA State Board of Pharmacy required.
