Job Description Salary: $25-$28 POLARIS TEEN CENTER is an esteemed residential treatment center serving adolescents ages 13-17 with severe mental health issues. We are currently looking for overnight shift (11:30pm-7:30am) LVNs to assist in supervising clients by doing rounds, and managing & coordinating medication with clients and treatment teams. JOB DESCRIPTION LICENSED VOCATIONAL NURSE Responsibilities : Provide direct care and nursing services to clients according to program policies and procedures, and/or individual treatment plan Maintains records pertaining to client medication count, weekly vitals, height and weight Manages urine specimen collection for all clients according to company policies and procedures Participate in multi-disciplinary treatment team meetings when mandates Oversees medication destruction of all discontinued medication according to company policy and procedures Responsible for preventing client abuse by adhering to all policies and procedures, attending periodic training, reporting suspicious and inappropriate behavior, and following mandatory abuse reporting requirements. Monitor chronic health conditions, respiratory distress, and seizures. Reports any client health concerns (e.g. unusual vital signs, weight, illness) to Program Director and Residential Supervisor Comply with all reporting requirements, biohazard program requirements, medical protocols, etc. Other duties as assigned. Minimum Requirement Current LVN license in California and 2 years prior nursing experience Strong organizational, interpersonal and decision making skills required. Must present a professional and compassionate demeanor in all interactions. Experience in residential nursing EMR savvy Reports To Registered Nurse Supervises N/A
