Nurse RN Adult Day – Brooklyn

SUMMARY: Provides professional nursing services for people supported served, following all OPWDD, agency and department standards of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING: Implements all AHRC health, medical and nursing policies and procedures.

Follow agency and department nursing policies, referring all questions to the Day Service Director of Nursing.

INFECTION PREVENTION: Monitors infection prevention in order to control communicable diseases and infections through identification and assessment, reporting to Nursing Director and AHRC Medical Director, and implementing appropriate protective and preventive measures.

Administers and/or reviews required initial TB screening of new individual we support as per the “AHRC Policy and Procedures for Control of Tuberculosis”.

Implements OSHA nursing standards as found in the AHRC Infection Prevention Policy.

Utilizes “Monthly Infection Control Compliance Check List” and the “Site Specific Plan” in liaison with the facility director to maintain OSHA Blood-borne Pathogen Guideline standards.

Provides site-specific OSHA required training annually.

MEDICATIONS: Follows ADS policy as found in the ADS Medication Policy & AMAP Policy Follows all applicable state and OPWDD nursing regulations pertaining the medication administration, and will be specifically guided by the “OPWDD Medication Administration Manual” which is used to instruct assistant medication administration personnel (AMAP).

Supervises all medication and treatment administration to people supported during the program hours.

Insures that medication orders, medication administration records (MAR) and prescriptions are written accurately, up to date/renewed and appropriately documented.

Supervises the care and storage of all medications, maintaining safety and security as necessary.

Monitors effectiveness of medications, making periodic checks that people supported are taking their medications as prescribed.

Informing parents/guardians/physicians of any concerns based upon your observations of the above.

Will complete and/or review “Annual Self-Medication Evaluation” for people supported that receive medications during day program hours, and implement with the goal of a person’s independence for medication administration.

AMAP: Will follow the regulations of OPWDD, the policy of AHRC, and the day program specific AMAP guidelines in all matters pertaining to AMAP supervision.

Completes initial certification process, re-certifies and de-certifies AMAP’s as per the AHRC AMAP Policy.

Conducts an annual in service specifically for AMAP’s in the program.

Provides oversight, training and supervision in the event of medication errors.

Conducts periodic direct nursing supervision all AMAP certified staff.

Communicates to AMAP staff that “on-call” nursing assistance by an RN is available in the event of the absence of the facility RN OTHER NURSING DUTIES Attends staff meetings and case conferences as required, presenting relevant medical and nursing data concerning individual’s health status and needs.

Reviews the individual’s incoming annual physical examinations for changes in health status.

Informs direct care staff of any medical precautions concerning people supported.

Completes initial and annual Health Emergency Profile assessments and Ready to Go forms for each individual.

Reviews Plans of Protective Oversight (POP) for accuracy and updating as needed.

Reviews incoming medical records, especially required annual physical examination.

Stamps and files all PHI as required.

Assists in obtaining required medical screening from family/residence and health care providers.

Communicates medical and nursing issues with parents/guardians by case note or documented phone call.

Communicates referrals for routine and/or special medical services to client CSS, MSC, family or residence.

Utilizes resources in the community for medical and health related services as appropriate.

Advises on diet and food safety/swallowing problems in coordination with nutritionist.

Reviews Dysphagia information as needed.

Provides competent nursing care and treatment for emergency first aid.

Makes referrals to hospitals, ER’s, or physicians for further intervention, as needed.

Supervises first aid care rendered by non-nursing staff.

Maintains inventory of all medical/nursing supplies for facility, ordering supplies as needed according department guidelines.

Provides timely and appropriate Incident Reporting, following AHRC guidelines, of all incidents.

Teaches applicable health curriculum to people supported on relevant issues that may arise.

Conducts annual in-services for staff following in-service guidelines and any other in-service’s that are necessary or may be requested by either the Director of Nursing or Program Director.

Keeps an attendance record of all in-services given.

Assesses with the program director whether a sufficient number of program staff is certified in First Aid, CPR and AMAP, providing appropriate nursing supervision to provide the best medical emergency coverage.

Other duties as may be assigned by Director of Nursing or Facility Director.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Must be a licensed Registered Nurse in the state of New York.

Minimum one year experience workingwith people with intellectual and developmental disabilities.

Bachelor Degree or a degree from a Hospital based program in Nursing, is required Experience in the counseling of patients and their families.

Ability to establish and maintain effective and therapeutic relationships with people supported, their families, your staff and outside agencies.

Ability to keep precise, accurate and timely medical records.

Ability to write and speak effectively.

Ability to fulfill the duties and expectations of a professional registered nurse.

Must possess basic computer skills (MS Office, E-mail, Internet) LANGUAGE SKILLS Ability to read, analyzes, and interprets professional periodicals and journals, technical procedures and governmental regulations.

Ability to write reports, business correspondence, and procedure manuals.

Ability to effectively present information and respond to questions from groups of professionals, managers and the general public.

MATHEMATICAL SKILLS: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

REASONING ABILITY Ability to define problems, collects data, establish facts, and draw valid conclusions.

Ability to solve practical problems and deal with a variety of issues that may occur in ADS facilities.

Must possess the ability to make independent decisions and handle stressful situations when circumstances warrant such action.

CERTIFICATES, LICENSES, REGISTRATIONS Must possess a current license/registration to practice registered nursing in the State of New York.

Current CPR certification.

OTHER SKILLS AND ABILITIES Ability to deal tactfully with personnel, residents, family members, visitors, government agencies and the general public.

Ability to provide leadership and to work harmoniously with and supervise professional and nonprofessional personnel.

Ability to be patient, enthusiastic, and confident when handling people supported, understanding their maturity functional level.

Must be knowledgeable in the field of MR and possess a good understanding of OPWDD regulations.
