Nurse Practitioner/Physician Assistant – Primary Care

Position Summary: The Advanced Practice Clinician-Primary Care will manage a growing panel of Medicare patients.

They will be responsible for routine primary care, including in clinic, at patient’s home and conducting telemedicine visits.

They will work with case managers, social worker extenders, other members from various clinical teams, and their supervising physician to provide high quality multidisciplinary care.

They will have access to advanced technology, including a robust health plan portal, Epic EMR, and a proprietary decision support tool that synthesizes data from a variety of sources into actionable alerts that drive care.

General Duties/Responsibilities (May include but are not limited to): Provides primary care for Medicare members in the patient’s home and telephonically Performs initial history and physical examinations and “Jump Start” assessments Orders, administers, and interprets appropriate diagnostic tests.

Performs diagnostic and therapeutic procedures such as administering IV, wound care, etc.

Administers vaccinations.

Communicates test results to patients and families.

Develops care plans and ensures all patients have advanced care planning.

Educates patients on various health related topics, such as disease management, medications, healthy lifestyle, end of life care, etc.

Maintains a customer-driven passion for excellence and commitment to innovation and implementation of ideas that improve the healthcare of the patient population.
