You will see scheduled patients in both a telehealth and event-based setting to administer Medicare prevention and wellness services.
Throughout the day there will be opportunities to act as an advocate and engage with people at the event who are unaware of these services and the health benefits of such.n n Together, Out into the Community, We Gon n We are looking for a full time Nurse Practitioner to join our team that works both virtually AND community based in the Los Angeles, San Diego, Sacramento, and San Francisco area.
This is a hybrid role that will start with a primary focus on telehealth and transition to include community-based events.
In this role, you’ll work with Medicare Advantage plans and their members to close care gaps as they attend our events both online and in person.n n These appointments are pre-scheduled to complete Annual Wellness visits with Health Risk Assessments.n n Services provided at the visit could include Diabetic Retinopathy Exams, A1c blood test, microalbumin urine test, and Care of Older Adult Measures (Advanced Care Planning, Functional Assessment, Pain Assessment, Medication Review).
Unlike a clinical or hospital setting, you’ll work with communities and patients that are currently healthy and looking to prevent items before they become major issues!n nHealth plan members value this service because they receive a comprehensive personalized prevention plan outlining their roadmap to optimal health and 1:1 time with a caring, trained nurse practitioner.
Additionally, they receive valuable education on their health conditions, coaching, and detailed medication reviews so they best equipped to manage their health.
The health plan covers these services at no cost to the member.n nMedicare Advantage plans value us because we have been able to activate members who were unresponsive to other offers to receive these services in other settings.
Also, we provide an excellent patient experience to their members, focus on the whole member by identifying barriers to wellbeing/ health, and address challenges related to each member’s social determinants of health.
Our work allows plans to reach their STARs goals which are key in their ability to remain a top-performing plan and retain members.n n What You’ll Need To Be Successful In This Rolen State NP License (Multi-state Licensure a plus!) Minimum 1 year experience Ability to work 5-6 days per week, as scheduled in advance Attention to accuracy and detail as you’ll work across multiple computer systems and platforms ANCC or AANP board certified required Flexibility to work both remotely and community based Willingness to commute in a company vehicle or by personal vehicle Ability to work autonomously and adapt to changing environmentsn n Compensation:n $400 per day for Telehealth visits $500+ per day for community-based events n Life Line Screening is proud to be an equal opportunity employer.
