Requirements and Qualifications:
Master’s Degree in Nursing.
Board certification and license.
Outstanding observation and analytical skills to diagnose and treat/manage patient effectively.
Excellent verbal, written and interpersonal skills.
Job Summary:
Provide clinical care and consultation.
Prescribe necessary treatment within scope of practice and assess patient’s clinical problems and healthcare needs.
Prescribe medications in compliance with established medical protocols and in compliance with government agencies and guidelines.
Educate patients on preventive care and health safety issue.
Evaluate and treat patients in the skilled nursing facility setting or post-discharge at the patient’s residential facility or private residence.
Perform annual wellness exam for senior Medicare patients at residential facility or clinic setting.
Cover for other primary care physicians when necessary.
Evaluate and treat patients independently.
Obtain patient’s medical history and document in EMR.
Create progress note for each patient encounter. A progress note will include Subjective, Objective, Assessment, and Plan sections, and must be completed after each examination.
Promote a friendly, warm, and exceptional customer experience.
Secures patient information and maintains patient confidence by completing and safeguarding medical records; keeping patient information confidential and abiding by all HIPAA rules and regulations.
Prescribe medications and treatment regimens as appropriate based on assessed medical conditions.
Any additional duties as assigned by Medical Director, MDs, and Director of Operations.
