Nurse Practitioner- California

This position will require traveling to members homes. It is not a remote position. 

Position Overview:

The Clinician/Provider will have experience working in multidisciplinary teams, the ability to practice autonomously with expertise in health conditions and management of health related conditions and the desire to positively impact member healthcare outcomes. Signify Health is focused on improving the quality of life for health plan members through effective care coordination. 

In this position, the Clinician will utilize their advanced clinical knowledge and expertise to complete in-home and/or virtual comprehensive health evaluations, capturing an accurate picture of the member’s health including all current, chronic and historical conditions, identifying gaps in care and assisting to develop a plan to manage their health, and uncovering clinical and social determinants that contribute to the management of the member’s health. 

The clinician must have advanced medical knowledge, ability to evaluate the results of questions asked, explain,  perform and interpret various lab and screening tests, analyze potential contraindications regarding medications and interactions with over the counter drugs and supplements, ability to determine diagnoses based on supportive evidence and provide additional assessments and determinations of health issues. The clinician will also be evaluating how the assessment findings impact the member’s health, identify risk factors that may exist and how this may impact their current health and need for further evaluation with referral to the health plan’s case management, primary care provider, and/or notifying adult protective services.   

 

Qualifications:

  • Nurse Practitioner
  • Physician Assistant

Minimum Education/Licensing Requirements: 

  • Graduate of a college of medicine or an accredited nurse practitioner program or accredited physician assistant program whose graduates are recognized as candidates for licensure by the regulatory authority issuing the license
  • Physicians must have completed at least one year of continuous post-graduate training in a single accredited Internship, residency or fellowship program (Fellowship pertains to foreign graduates who have not completed residency in the U.S).
  • Provide a Collaborative Agreement/Written Protocol for Nurse Practitioners or Physician Assistants at the time of initial and recredentialing where applicable
  • Clinician with license in good standing in the state where work is being performed
  • Nurse Practitioners and Physician Assistants must have current board certification 
  • Because of the advanced clinical knowledge and expertise necessary to perform the comprehensive home health evaluations, registered nursing degrees are not sufficient for this position

Essential Skills/Experience:

  • Experience in utilization review, concurrent review and/or risk management a plus 
  • Bi or multilingual ability a plus
  • Minimum of 2 years’ clinical experience working with complex adult populations preferred
  • Minimum 1 year of experience conducting Health Risk Evaluations preferred
  • Experience performing chart audits and educating peers on best practices preferred
  • Adult, Family Medicine or Geriatrics experience preferred
  • Awareness of issues related to communication, cognitive or other barriers
  • Strong critical thinking skills and analytical abilities
  • Ability to make physical and mental health assessments of the members
  • Ability to conduct and analyze the results of various medical tests
  • Ability to make independent judgements and recommendations regarding the member’s health care
  • Strong organizational skills 
  • Strong computer skills with working knowledge of Google Suite (Google sheets, Google docs, Google slides, Google forms) and various computer software programs 
  • Competency with iPad technology
  • Ability to work remote and autonomously 
  • Ability to effectively present information in writing and verbal communication by responding to questions from members, providers, Signify Health staff and external stakeholders benefiting the member’s health

Essential Job Responsibilities:

  • Clinicians must utilize their license and/or certification and their advanced clinical knowledge and expertise to conduct regular health evaluations in members’ homes or via virtual platforms. Health evaluations include a comprehensive review of the member’s health history, family medical history, allergies, medications taken, review of screening and immunization history, and other SDOH factors that may impact a member’s health, and a clothed physical exam with height, weight, and vital signs measured. The Clinicians must make physical and mental health assessments throughout the evaluation, must analyze test results, and must make informed recommendations regarding the member’s health care needs
  • Clinicians are expected to spend the majority of their work time performing duties for which their advanced medical certification is required
  • Conduct ancillary screenings including but not limited to lab work (A1c, FOBT, CKD), digital eye exam (DEE), peripheral arterial disease (PAD) screening, and bone density screening as needed
  • Partner with Signify Health, the health plan, and providers to help coordinate both internal and external resources to meet the member’s needs as identified and evaluated by the Clinician
  • Prepare written reports and assessments with the Clinician’s analysis and evaluation of the member’s health situation and recommendations regarding the member’s health care
  • Conduct peer reviews via Signify Health chart audit tool monthly
  • Conduct ride alongs as requested by regional clinical manager lead
  • Possess a strong knowledge of clinical procedures, standards, and quality control checks
  • Possess a strong knowledge of medical conditions, interventions, treatment and medications that is consistent with their certification and/or license
  • Possess strong knowledge of vaccination and preventative screening guidelines
  • Provide members education regarding health evaluation findings
  • Maintain professional appearance (lab coat required) with photo ID badge visible 
  • Perform other related duties and responsibilities as assigned
  • Provide feedback to Signify product and tech teams through email, surveys, focus groups or other mechanisms.

Additional Job Responsibilities:     

  • Required to participate in virtual and in-person staff meetings, conference calls and other meetings, as requested by Signify Health
  • Maintain working equipment and supplies, proactively ordering replacement supplies, as needed
  • Required to attend virtual and in-person training sessions to acquire/enhance skills related to programs offered
  • Complete projects/tasks as requested by your supervisor
  • Performs other incidental and related duties as required
  • Accommodate ride-alongs as requested.

  • Position requires travel to member homes via car and ability to enter homes with steps or other barriers 
  • Travel overnight required as needed
  • Ability to obtain additional state licensures as needed and directed by Signify Health
  • Members’ home environment conditions vary. Clinicians must be able to see members in homes with pet dander, smoke, and other environmental conditions.
  • Use iPad and diagnostic preventive services (DPS) equipment effectively 
  • Work effectively with frequent interruptions
  • Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (includes iPad, medical supplies and scale), required
