Ranked as one of the best hospitals to work at for the past three years, this medical center is searching for an excellent communicator, highly organized, knowledgeable Nurse Director of Case Management.
With over 800+ hospital beds, the Nurse Director of Case Management is responsible for care delivery, care coordination systems that support quality patient care in an efficient and timely way. This non-profit hospital has numerous leading specialty centers to support such as: cancer, heart, rehabilitation, orthopedics, neurology, and trauma.
The Nurse Director of Case Management will be responsible for ensuring that all patients are receiving proper care, building relationships within community outreach programs, and maintaining key metrics; such as Length of Stay, Readmission Rates, Patient Outreach and Education, and financial performance. The Nurse Director of Case Management will report directly to the Chief Nurse Officer and will provide leadership to Case Managers. The Nurse Director of Case Management will comply with budgets, maintain staffing resource levels, and hold staff accountable for performance goals.
The Nurse Director of Case Management will oversee two campuses that admit about 40,000 patients on average, while collaborating with Registered Nurses, Case Managers, and Registered Nurse of Utilization Management. The Nurse Director of Case Management has expert knowledge of utilization management, is fiscally responsible and meets current needs of the community, while providing a top-of-the-line patient experience.
