We are hiring a National Clinical Change Manager – ISNP.

Responsible for assisting in reviewing current and necessary new clinical workflows to improve clinician performance ultimately impacting patient care and product/program KPIs including patient admissions and readmissions.

Assists with change management, project management, product implementations and execution of plans for clinical products/programs.

Identifies subject matter experts in the markets, develops and leads small work groups, leads pilots as necessary, provides ongoing communication and feedback, reassess the need for additional change in relation to clinical products/programs..

Primary duties may include, but are not limited to:.

Assists in developing standardization/workflows of growing clinical products/programs nationally to meet key performance indicators and expectations..

Assists in championing necessary changes, leads small work groups, attends local market meetings to assist with change acceptance and communications..

Rolls out national best practices for clinical products/programs..

Collaborates with leadership to lead clinical products/programs..

Develops implementation plans for key clinical initiatives for clinical products/programs..

Performs project management for clinical product changes and ensures timely implementation of changes throughout the clinical product/program..

Assesses current workflows, completes needs-based assessments, and improves current workflows resulting in improved satisfaction for clinicians delivering care for clinical products/programs..

Rolls out national clinical best practices for clinical products/programs..

Develops small workgroups and pilots for clinical products/programs to drive changes and gain clinician buy in to promote change acceptance and improved processes..

Qualifications.

Requires MA/MS degree and a minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant; or any combination of education and experience, which would provide an equivalent background.

Current unrestricted Nurse Practitioner or Physician Assistant license in applicable state(s) required.

National certification from an accredited organization required..

Preferred Qualifications.

MA/MS in Nursing, Public Health, or Healthcare Administration preferred..

5-7 years of experience leading clinical change strong preferred..

Experience with ISNP product preferred..
