The Medical Asst
– Preventive Care maximizes physician practice ability to complete Annual Wellness Visits (AWVs), as well as on-site work to close care gaps thru the year and as ad hoc needs arise.

The role will report to the Supervisor Scheduling Advocates with input from Jefferson Community Physicians administrative team.Meets all standards of the Patient Satisfaction Initiative Criteria in order to make Abington
– Jefferson Health (AJH) the hospital of choice for Patients and Employees.Using existing data analytic tools from payers and internal sources to identify care gaps, initiate outreach with direct patient contact via telephone or electronically to close those gaps.Using data tools to screen Medicare patients for those in need of annual wellness exams and initiate the completion of wellness exams based on protocols.Qualifications3 years of clinical experience as a practicing MA required.High school diploma or equivalent.
