LMFT – Homeless Healthcare Collaborative

Responsibilities

UCLA Health seeks a full-time Licensed Marriage and Family Therapist (LMFT) for the Homeless Healthcare Collaborative, an innovative alternative health care delivery program to provide care to persons experiencing homelessness (PEH).

The LCSW will provide services in the community to PEH in a mobile clinic.

You will report to the Medical Director and Operations Director.
The LMFT will provide direct social work services, including assessment, intervention, recommendations, insurance enrollment, social service assessment, and social service referrals to people experiencing homelessness and housing instability in the community setting.

The LMFT will be responsible for social service needs assessments and referrals, community-based referrals, insurance enrollment and verification, providing psychiatric resources/referrals, and safety planning with individuals and families experiencing homelessness.
This position will also include sharing the responsibility for driving the Community Care Van.

Qualifications

Required:

Must possess a current license (L.M.F.T.) with the California Board of Behavioral Science.

Excellent oral and written communication skills.

Strong relationship building skills.

Excellent problem solving ability.

Demonstrated skill in psychosocial assessments of patient / families, clinical interventions, and treatment planning with patients and families.

Demonstrated work with interdisciplinary teams
Must have basic computer knowledge and word processing skills.
Must carry beeper during normal work hours and respond to pages in a timely manner.
Must operate telephone, fax, and copy machines.

Preferred:

Post-masters experience in a healthcare setting is highly desirable.

A Master’s Degree in a relevant field.
