Benefits (Full-time): $2,000.00 Sign on Bonus Medical/Dental/Vision Insurance Life Assistance Program Flexible Spending Account (FSA) Paid Holidays Paid Vacation/Sick/Bereavement leave Credit Union Benefits and Discounts $750.00 Referral Bonus Program Retirement Plan (403B) Employer Sponsored Life Insurance Direct Deposit Hands-On Training Reliable Schedule LVN E ssential Responsibilities: Promotes a friendly, comfortable, and safe environment Provides and preforms nursing care as directed by Momentum and ICF/DD-N’s policies and procedures Passes medications and monitors/reports any changes in each persons health/condition Assess signs and symptoms indicating changes in persons supported health/condition Assists RN and Administrator/QIDP with new hire and ongoing staff training of vital signs, medication administration, and other training as needed Assists with scheduling doctors appointments and ensure specialist referrals and appointments are properly scheduled Attends and transports people supported to appointments in company vehicle as directed Prepares peoples supported charts for doctor rounds Assists physician on rounds; receives and records doctors orders Maintains all data collection forms, notes, charts, and other documentation for each person supported Performs other duties as assigned to meet department and/or business needs Education and Experience: Must have a current LVN license issued by the State of California 6 months of work experience as an LVN; preferred Previous experience working in ICF/DD-N setting; preferred Previous experience as a medical assistant; preferred Skills and Knowledge: Strong interpersonal, teamwork skills, and willingness to work with different people and departments Strong ability to communicate and work professionally and cooperatively with others Excellent time management skills Knowledge and/or willingness to learn electronic medical charting; required Requirements: Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States Ability to run, push, pull, bend, squat, kneel, walk, stand and sit for long periods of time, and lift up to fifty (50) pounds Valid and current proof of legal right to drive in California; required Sign on Bonus will be paid in two payments. First payment will be made after 90 days of active employment and second payment will be made after 6 months to active employees in good standing.
