Job Description Definition: The Licensed Vocational Nurse (LVN) I/II provides a variety of administrative, clinical nursing, health education, and outreach services within the community health center. Basic skills for physical assessment include observation, auscultation, palpation and percussion. Basic mental status assessment includes observation of general behavior, appearance and attitude, collection of data relative to sensorium or orientation to time, person and place, and assessment of recall ability. When the LVN notes a problem, complication or abnormality, he/she must notify the physician or Registered Nursing Supervisor under whose direction the licensee functions. The LVN cannot independently evaluate or synthesize data. Reports to: Direct & Administrative: Registered Nursing Supervisor or Clinic Administrator Professional/Technical: Registered Nurse Supervisor in consultation with the Licensed Medical Provider Lead Duties : For LVN II, may lead a team of Medical Assistants, Team Clerks, Community Health Workers, and other unlicensed assistive personnel as appropriate. Responsibilities: Performs, under the direction of the RN Supervisor or Program Manager, delivering technical nursing services in caring for acute or chronically ill patients, including: Assists provider with examinations or special procedures, such as lumbar punctures, sigmoidoscopies, etc. Administers medications orally, topically, and by injection. Provide nursing services, including routine screening services (e.g., EKG, hearing and vision, and immunizations). Communicates information to the RN Coordinator or Clinic Administrator through observation of the patient’s physical condition and behavior, signs and symptoms of illness, and reactions to treatment and communication with the patient and other team members. Contributes to evaluating individualized interventions related to care or treatment plans established by the medical providers. Executes interventions in accordance with the patient’s care or treatment plan established by the medical providers to ensure the patient adheres to and completes their medical treatment. Serves as the nursing team leader and coordinates the activities of Medical Assistants and Team Clerks in the absence of the Registered Nursing Supervisor or Clinic Administrator. Initiates emergency procedures and coordinates the activities of Medical Assistants and Team Clerks until a provider or registered nurse arrives. Participates in planning and executing emergency code drills. Performs CPR as needed. Assembles appropriate and required chart data and gathers basic physical health problems and barriers to medical treatment adherence. Maintains required logs and statistics (e.g., the Los Angeles County Department of Public Health). Work with medical providers to review incoming reports from laboratories and report positive cases to providers and LA County DPH. Provides skilled nursing care for primary care patients, including but not limited to administering medication, treatments, and screening procedures as ordered within the scope of practice of the license. Applies injectable medications as indicated by the medical provider. Participates in the in-service training of unlicensed assistive personnel and the orientation of new team members. Contributes to developing and implementing a teaching plan related to patient self-care and provides education and reinforcement in increasing their primary care treatment. Participates in patient follow-up activities for missed appointments and abnormal laboratory results and maintains patient tickler files. Increase the access and utilization of medical services by assisting patients in eliminating barriers to care, such as forgetting to take their medication, confidentiality issues, etc. In addition, it promotes early and ongoing interventions for treatment adherence. Participates in routine audits of patient medical records to determine the appropriateness of nursing care, documentation, and follow-up plans. Perform other duties as may be required or assigned by the RN Supervisor, Clinic Administrator, or Director of Nursing Services. The MIS Patient Information System access level is 2.
