The Licensed Clinical Social Worker will provide a full range of social work services in collaboration with other CSMC staff to meet the psycho-social assessments, crisis intervention, Care Coordination, transitions management and discharge planning needs of patients and families. Successful candidate will provide, among other primary social work functions, supportive counseling, community resources, and coordination of family conferences.
