Job Description POSITION SUMMARY Under the direction of a CMO/Clinic Manager, provides care to patients in a knowledgeable, skillful, consistent, and courteous manner consistent with scope of practice and following established policies and protocols. The LVN contributes to the provision of quality nursing care through techniques that support and achieve positive outcomes in patient care. REQUIRED SKILLS AND QUALIFICATIONS Education : • Graduate of an accredited LVN program Experience : (Required) • A minimum of 2 years actual vocational level nursing experience, such as: emergency room, medical office (pediatric, OB/GYN, or other specialty), or hospital care. • Able to travel to multiple sites within service area. • Excellent oral and written communication skills • Ability to function independently and as a team member • Proficient computer skills – Word, Excel, Outlook • Bilingual English / Spanish (Required) • PMS – Practice Management System skills (Preferred) • E.H.R- Electronic Health Records (Preferred) Licensure/Certification : Employees are responsible for maintaining individual certifications as required by job function or by law and provide verification and recertification when requested by management. • Graduate of an accredited LVN program • Valid LVN license • Current CPR certification PRINCIPLE ACCOUNTABILITIES. Duties and Responsibilities: Personal – Team – Organization Clinical Responsibilities • Organize getting the morning started – trouble shoot if there is a delay or snag, expedite as needed to ensure that patients are in the room by 8:30 • Limited, basic triage of patients, consistent with scope of practice and in collaboration with lead clinician, to expedite waiting patients or overbook walk-in patients based on medical urgency – Start albuterol treatments on wheezing patients – Perform EKGs and inform provider on patients with chest pain – Determine if patients should go directly to ER (ex. Severe pain or wounds) • Help make specialty appointments with patients under the direction of the provider • Communicate with referral department on status of referral and extend referrals if needed • Eventually help clinic to design a system to systematically and rapidly inform patients of normal results as well. • Conduct similar duties for Telehealth/Telephone Encounters • Call patients regarding abnormal imaging, lab results, and follow-up plan/treatment based on provider instructions (create telephone encounter) • Assist on-site medication management, including IM and IV injections within the scope of practice. • Take calls from outside pharmacies and communicate with/or send telephone encounters to providers as needed – for refills and medication changes • Eventually help clinic to design a system to systematically and rapidly inform patients of normal results as well. • Oversight of rapid HIV testing, including quality assurance and competency training for MAs • Basic Wound care according to written protocol • Insulin/Glucometer/Diabetes Education • Provide guidance and counseling to clinical staff to ensure efficient coverage of clinical area. • Assist provider with minor surgery/as needed • Follow policies and procedures relating to patient flow, customer service and compliance with regulations pertaining to clinical area. • Directly responsible for tracking, ordering, and maintain clinical supplies and minor equipment. • Communicate with front and back office to facilitate patient flow. Labs: • COVID-19 – Swabbing patients (symptomatic and asymptomatic) – Vital signs for patients in tent – Maintaining tent log – Calling patients with result – Managing tent flow – Translation • Oversight and organization of lab letters • Oversight of Title X counselor to call Chlamydia patients within 24 hours • Facilitate CMR for STD Reporting/Report CMR/STD to the state/keep up with the log book. • Assist providers to receive and send reports to the hospitals • Facilitates clinicians response to critical labs by coordinating charts and calling patients as needed Additional Duties • Assist the site clinical lead to promote a team environment in which high quality care is provided in a manner that is efficient, responsive to patient’s needs, user friendly safe and cost- effective. • Provide Training for new Medical Assistants and Retrain MAs as appropriate. • Monthly Inventory – Emergency Crash Cart • Help with physicals along with MAs, especially with the pediatric patient physicals and Annual Wellness Visits • Ens
