Position Open to Registered Nurse (RN) or Social Worker Job Summary: The Home Health Discharge Liaison assumes responsibility for receiving and processing referrals for Home Health Care Services, consistent with Saint Francis Hospitals mission and philosophy.
Education: Completion of a Masters in Social Work or completion of a State approved Registered Nurse program.
Licensure, Registration and/or Certification: Valid State of Oklahoma Social Work license or valid State of Oklahoma Registered Nurse license, Valid State of Oklahoma drivers license, a motor vehicle report that meets SFHS guidelines and proof of vehicle liability insurance in the amount required by SFHS guidelines.
Work Experience: Minimum of one year with Home Health experience preferred.
Previous experience in discharge planning and/or Medicare regulations preferred.
Knowledge, Skills and Abilities: PC software skills.
Good interpersonal skills.
