HIV Community Health Educator (Hybrid Role, both on-site & remote)

The HIV Community Health Educator supports the Ending the HIV Epidemic (EHE) Project at Eisner Health. The primary goal of the HIV Community Health Educator is to effectively engage and educate patients, and community members, on HIV infection risk, testing, as well as PrEP awareness. The HIV Community Health Educator will primarily be based at a partner’s location, but will work collaboratively with health providers, other members of the Eisner Health’s care team and community resource representatives to engage the patients in HIV testing and counseling, and coordinating their access to PrEP as appropriate and to HIV treatment as needed.

ESSENTIAL DUTIES

1. Engage in recruitment efforts to make the project services known and available to patients and community members (participants).

2. Conduct educational presentations (online and in-person) on HIV infection risk, testing and PrEP at the collaborating partner site primarily, but also at Eisner Health clinics and other community locations as requested.

3. Conduct risk assessments for interested participants and provide rapid HIV testing and counseling.

4. Support access and coordination of PrEP and HIV treatment for patients as needed.

5. As needed, assess the participant’s readiness to change and assist him/her in following a plan of action to obtain and maintain PrEP and/or HIV therapy.

6. Support health behavior change utilizing motivational interviewing and trauma informed care practices.

7. Directly and regularly communicates with providers on participant’s medical care as appropriate

8. Monitor treatment adherence.

9. Serve as primary contact to the participant for access to needed services.

10. Connect students from the collaborating partner(s) with primary care services at Eisner Health.

11. Maintain accurate monitoring and evaluation records.

12. Support the completion of reports to funder as directed by supervisor and/or Development Manager.

13. Perform other duties as assigned.

POSITION REQUIREMENTS

1. Bachelor Degree required

2. Two years of experience in community health or social service setting strongly preferred

3. Previous experience in HIV prevention education preferred.

4. Effective communication and interpersonal skills

5. Demonstrated public speaking and persuasion skills

6. Ability to work collaboratively in interdisciplinary team setting.

7. Proficiency in Microsoft Office Suite products.

8. Experience with Electronic Health Records preferred.

9. Valid driver’s license and willing to drive to Eisner sites and communities where patients live

10. Spanish speaking preferred
