HEALTHRIGHT 360: Systems Navigator – Prcs

Job Overview Systems Navigators (SN) are responsible for a comprehensive array of case management and advocacy services to the Los Angeles County Department of Probation Post Release Community Supervision (PRCS) population.

Primary functions of the SN are to assess PRCS population needs and providing assistance with obtaining needed documentation, services and linkages germane to their reentry efforts.

Key Responsibilities Services Responsibilities: Responsibilities include telephonic and/or face-to-face assessments (assessing the participants’ needs) and assist the client in identifying what public assistance they are eligible for and assist the client with completing necessary paperwork required to receive public assistance.

Additionally, they will assist the client in obtaining birth records and/or identification and Social Security Cards.

Service provision to PRCS participants includes but is not limited to: identifying and procuring ancillary services, auxiliary funds requests, referral to housing resources, maintaining project documents and records, and ongoing communication with the LA County Probation personnel.

The PRCS SN case manages the participant from the intake/assessment process through the final stages of acquiring benefits/documents and housing.Administrative Responsibilities: The position requires ongoing data entry of progress notes, filing, case management plans and quality assurance monitoring of participant documentation/benefits/ancillary services acquisition progress.

Ensures that subcontractor participant files contain all pertinent documents.

Assure confidentiality of all incoming and outgoing client data.

Generate reports pertinent to client outcomes and services rendered.General Responsibilities: Dependable automobile and insurance, registration and valid California Driver’s License.

Education and Knowledge, Skills and Abilities Education and Experience Required:Graduation from high school or equivalent.Two years of paid experience at an agency providing services to a criminal justice population.Cannot be actively on probation or parole for past three years.Valid California Driver’s License, reliable vehicle, and willingness to travel (mileage allowance provided).Culturally competent and able to work with a diverse population.Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.Desired:Bachelor’s Degree or Alcohol and Other Drug certification.Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency.Background Clearance Required:Must undergo a background investigation and criminal records check to obtain clearance to work within criminal justice institutions.

Final approval is contingent upon clearance by the CDCR.Must not be on active parole or probation.Knowledge Required:Case management principles, planning, resource development, benefits entitlements (SSI, General Relief, Vocational Rehabilitation), documents acquisition (birth certificates, criminal history, SSA cards).Skills and Abilities Required:Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data.Professionalism, punctuality, flexibility and reliability are imperative.Excellent verbal, written, and interpersonal skills.Integrity to handle sensitive information in a confidential manner.Action oriented.

Strong problem-solving skills.Excellent organization skills and ability to multitask and juggle multiple priorities.

Outstanding ability to follow-through with tasks.Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility.Strong initiative and enthusiasm and willingness to pitch in whenever needed.Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations.Able to work within a frequently changing project scope while maintaining overall direction and structured priorities.Desired:Bilingual.In compliance with the California Department of Public Health’s mandate, by September 30, 2021, all employees must be able to provide proof of COVID-19 vaccination.
