Health Educator I

Overview

The Health Educator provides Comprehensive Prevention Education and outreach to diverse populations in a variety of settings while promoting the mission of AltaMed Health Services. Provides support to Project Manager with implementation of curriculum and work plans for several grants

In addition, s/he will work with the health education team to conduct training for AltaMed staff and community members. S/he will maintain the health education distribution center as well as aid in the development of a health education center at each AltaMed site.

Responsibilities

  • Provide health education to patients/clients/community members as directed by the Program Manager.
  • Aid in the development of a health education center at AltaMed sites.
  • Participate in staff meetings in order to provide health education technical assistance and guidance organization-wide as requested.
  • Review health education materials for content and cultural appropriateness.
  • Identify and research additional educational resources, as needed, to support and reinforce healthcare and health education messages.
  • Participate in community outreach, education health fairs, etc.
  • Assist in the development of the health education work plan.
  • Serves as facilitator in resolving patient concerns.
  • Implement Health education objectives and strategies within the clinic and throughout the community.
  • Support implementation of marketing strategies to increase targeted patient enrollment.
  • Provide outreach on AltaMeds services and specific health topics including reproductive health education.
  • Develop new and continued relationships with schools, shelters and community serve organizations; implementing comprehensive reproductive and other health education workshops as needed.
  • Act as a liaison to school personnel and community collaborators to ensure effective partnership and cooperation between project (grant) activities, requirements and needs.
  • Provide prevention education in schools, community based organizations, churches, etc.
  • Provide patients with education and necessary referral sources.
  • Work with Program Manager to develop culturally competent and relevant education materials for presentations.
  • Attend and participate in community meetings and collaborative efforts addressing health related issues.
  • Attend training and conferences scheduled and coordinated by the California Family Health Council (CFHC) as directed.
  • In collaboration with Project Manager, collect data relevant to the evaluation of prevention education and grant specific audiences.
  • Facilitate clinical linkages to clinics.
  • In conjunction with Program Manager and Health Education team may train and educate staff and student interns performing related work.
  • May work in collaboration with Program Manager recruiting volunteers, as needed in the area of operation
  • Create PowerPoint presentations promoting AltaMeds Health Services.
  • Implement other duties as assigned by the Program Manager.
  • Qualifications

  • Bachelors Degree in Science in Health Science with an emphasis in Community Health Education or related field
  • Minimum 2-3 years experience or internship in health care field or community outreach
