We are seeking an experienced, knowledgeable Case Manager to work directly in our Emergency Department, collaborating closely with Physicians to determine that most appropriate level of care for a diverse array of patients and medical conditions.

Summary of Essential Duties:
Provide case management services in collaboration with physicians, CSMC staff, and the Emergency Department Primary services include providing consultation regarding patient classification determination (inpatient or observation) Recommend alternatives to admission (non-acute resources, alternative levels of care) Work with the healthcare team to achieve optimal clinical, financial, and patient satisfaction outcomes Identifies and intervenes when plan variances occur Identifies practice patterns and quality issues that arise Initiates transition of care for patients who will be managed by the inpatient case managers on patient care units The schedule for this position will be three 12-hour shifts per week, 12:00pm
– 12:30am including weekends
Education:
Bachelor of Science, Nursing (BSN) required Master’s Degree, Nursing (MSN) preferred
License/Certification:
Current, unrestricted California RN License required BLS from the American Heart Association or American Red Cross required Certified Case Management RN preferred
Experience:
A minimum of 5 years of acute care nursing experience required A minimum of 2 years of case management experience required At least 3 of years of ED nursing experience preferred Emergency Department Case Manager experience preferred Experience using Interqual or Milliman required High level of competency performing medical necessity reviews with accepted criteria required
