Role: Enrollment Growth Director, ConcertoPACE of Los AngelesLocation: Los Angeles, CAStatus: Full-timeMission: To allow frail elderly people to age in the location of their choosing and continue to leadconnected, meaningful livesJob SummaryConcertoPACE of Los Angeles is looking for an energetic, mission-driven, and ownership-oriented individual to lead local marketing, community engagement/outreach, and enrollment for a new program dedicated to serving the healthcare needs of the frail elderly population.

This role is accountable for meeting enrollment growth goals of the market and interfaces with participants, families, caregivers, local community organizations, and state regulatory agency to facilitate that process.

Joining the leadership team at an early stage, the Enrollment Growth Director will shape and drive the brand and identity of ConcertoPACE of Los Angeles by leading and executing all community engagement strategies within the market.

The Director will be responsible for building a team of talented outreach and enrollment staff and facilitating an effective intake and enrollment process in partnership with clinical teams.

The primary measure of success for this role will be based on participant enrollment and associated pipeline development (e.G., outreached, referred, processing).

Other key indicators include relationships built with local community partners (who serve as potential referral sources), public awareness of ConcertoPACE of LA, and team performance and culture.

This partnership combines Edenbridge Health’s deep expertise in the PACE model of care with ConcertoCare’s experience in leading Medicare and Medicaid risk-bearing provider entities.Essential Duties And ResponsibilitiesThe Enrollment Growth Director is accountable for meeting performance targets on referral generation and census growth by:Leading community engagement and marketing efforts and; Overseeing enrollment and outreach team.

COMMUNITY ENGAGEMENT & GROWTH Grow enrollment pipeline by identifying potential referral sources and executing community engagement and partnership strategies; As program scales, build, train, and manage community outreach team to execute grassroots community engagement; In partnership with national PACE team:Develop, implement, and monitor multi-modal marketing campaigns that build awareness of ConcertoPACE LA’s brand; Provide input into marketing collateral needs and design; Establish and maintain technologies for execution, monitoring, and oversight (e.G., marketing campaign tools, customer relationship management (CRM) systems); Conduct research and maintain awareness of competitors’ positioning within the market to inform business strategies.

ENROLLMENT EXECUTION Successfully facilitate the enrollment of referrals into the program by following enrollment processes while identifying opportunities for efficiencyAs program scales, build, train and manage a team of intake coordinators, assessment staff, and Medicaid specialists to meet enrollment needs; Establish and monitor key performance indicators to ensure an efficient enrollment process that a) appropriately maximizes capture of qualified leads, b) overcomes common barriers to enrollment and c) minimizes near term disenrollment; Ensure enrollment processes effectively administer level of care assessments, financial eligibility, home visits, and handoff to interdisciplinary team; Maintain a strong working relationship with local and state regulatory agencies to ensure compliance with DHCS and CMS guidelines and policies; Maintain compliance with audit and reporting requirements including but not limited to Medicaid reconciliations and redeterminations.The Enrollment Growth Director may be tasked with other duties as assigned including but not limited to strategic projects and supporting internal communications needs of the market.QualificationsBachelor degree in a business or healthcare related field; 5+ years experience working in growth, business development, community partnerships or patient-facing sales in a healthcare context; 3+ years of supervisory experience with a track record of developing inclusive and high performing teams; Passion to serve the frail elderly; Strong interpersonal skills and excellent verbal and written communication skills; Internally motivated, team player, attentive to detail and performance oriented; Strong customerservice orientation; Proficiency in the use of customer relationship management (CRM) systems and MS Office; Ability to workin collaborative (as well as virtual) team environments; Ability to travel locally within the program services area.
