We are Compassionate Visionary Health Care, where passion and team spirit are nurtured and rewarded.PRIMARY FUNCTION: Supervisor is accountable for the coordination and management of the multidisciplinary Rehab Care teams; ensuring that patient care provided consistently maintaining the highest home health standards, continuity of care and services are delivered in the most efficient manner.POSITION OVERVIEW:* Supervisor is the corporate liaison to the CVH patient and patient’s PCP (Primary Care Provider).Works in partnership with clinical and administrative personnel throughout the agency.* Will collect and assess information regarding patients’ status throughout episode and makerecommend as to which patients will continue to receive care through CVH and/or which will bedischarged.
Will follow established Medicare guideline and company protocols when makingdeterminations to discontinue treatment: i.E.
unmet face to face encounter requirements, absenceof a PCP to direct home health care.* Oversee the submission and follow up of verbal orders, supplemental orders, additional visits, newevaluations and additional disciplines, via PCP.* The Supervisor or designee will review the plan of care for all patients.* Assists the Rehab Director as needed with development and approval of 485 within 5 days of SOCvisit.
Sign the plan of care and record other clinical personnel who reviewed it.* Oversight and coordinate the work of Schedulers, Authorization Coordinators, other administrativeoffice personnel, including reviewing work performance, motivating subordinates to work moreeffectively and identifying problems with performance in collaboration with the Director of ClinicalSupport, develop solutions to those problems, determining training needs to improve performance,initiating informal measures to correct performance issues and/or or making recommendations forformal discipline.* Supervise and coordinate the work of Field Clinicians, including reviewing work performance,motivating subordinates to work more effectively, identifying problems with performance anddeveloping solutions to those problems, determining training needs to improve performance,initiating informal measures to correct performance issues and/or or making recommendations forformal discipline.* Facilitate multidisciplinary case conference meetings with clinical and administrative personnel tocoordinate delivery of patient care.* Assist Rehab Director in the planning, facilitation and evaluation of in service and continuingeducation programs.* Comply with accepted professional standards and principles.* Perform other reasonable and relevant duties and activities as delegated by Director of PatientCare Service or Chief Operating Officer (COO).EDUCATION:* Possesses a therapy degree from a Baccalaureate, Master’s or Doctorate program approved by a degree from a Baccalaureate, Master’s or Doctorate program approved by an accredited organization.* Currently licensed as a Physical Therapist accredited organization.* Currently licensed as a Physical Therapist or Occupational Therapist or Occupational Therapist by the by the California Board of Physical Therapy.* Maintains a current BLS (basic life support) for Healthcare Providers or CPR/AED (cardio pulmonary resuscitation / automated external defibrillator) certification.QUALIFICATIONS:* Two (2) years’ experience within the last (2) years’ experience within the last five (5) years, in either a home health agency, primary five (5) years, primary care clinic or health facility; unless the individual has been previously approved for such care clinic or health facility; unless the individual has been previously approved for such employment by a program flexibility issued for the individual’s current position at the home health employment by a program flexibility issued for the individual’s current position at the home health agency prior to April 1, 1995.* One (1) year of One (1) year of recent Home Health experience.* Sufficient computer skills computer skills to operate and complete documentation in a timely manner on an Electronic Medical Record.* Sufficient knowledge of Sufficient knowledge of Medicare regulations to be and able to perform an OASIS assessment.* Sufficient knowledge of home health care operations, self Sufficient knowledge of home health care operations, self-direction, to be able to work, visit direction, to be able to work, visit patients and follow Agency procedures with minimal supervision patients and follow Agency procedures with minimal supervision.* Shows ability to organize and prioritize workload.
Able to case manage and coordinate care with other clinicians* Able to communicate clinical observations to physician.* Knowledge and expertise in clinical decision making and case management review and evaluation.* Sufficient background knowledge and expertise in clinical decision making for the patient making for the patient population assigned to him or her in the home health agency to meet the needs of his or her patients and to contribute to quality management review and evaluation.* Demonstrates excellent observation, verbal and written communication skills.* Ability to market and deal tactfully with customers and the community.Our outstanding clinicians and staff make CVHCare an outstanding company it is today.
