Continuity of Care Nurse Case Manager – Los Angeles, CA – Case Management

Responsibilities

Using your advanced practice nursing skills, you will assess, coordinate care and provide education for a diverse patient population.  This will involve collaborating and consulting with a multi-disciplinary clinical/professional team as well as patients and families to ensure safe and effective coordination of care. You will:

 

·Assess, coordinate, develop and implement individualized care plans

·Utilize evidence-based tools for risk stratification

·Perform utilization reviews, including authorization reviews

·Ensure delivery of concurrent and post-hospital care

·Initiate referrals, coordinate resource utilization, and evaluate post-acute care services

·Participate in performance improvement projects/processes

·Implement comprehensive DC plan for high-risk patients

Qualifications

We’re seeking a self-directed, assertive, creative problem solver with:

 

·A current, valid CA LVN license

·At least three years of acute hospital experience

·Two or more years of utilization management, case management or discharge planning experience in an HMO environment

·Team leading or management experience

·Systems planning and care management background in a high-volume setting

·Proven resourcefulness with strong communication, diplomacy, organizational, prioritizing and analytical skills

·Ability to work effectively and collaboratively with interdisciplinary teams

·Computer proficiency

·Understanding of the functions of a large university teaching hospital

·Ability to abstract and interpret medical information from patient records

·Familiarity with the laws, rules, and regulations regarding utilization review and discharge planning

·Ability to develop individualized case management plans that addresses physical, vocational, psychosocial, financial, and educational needs
