Description: JOB SUMMARYThe Behavioral Health case manager addresses the needs of patients who are referred by CCHC providers, based on an assessment of Social Determinants of Health (SDoH).
The BH Case Manager supports patients by directly linking them to community mental health and social service resources in order to achieve better health outcomes.ESSENTIAL DUTIES AND RESPONSIBILITIES Follows up to ensure appropriateness of referrals; Supports the integrated care process; Serves as one ofthe communication links between medical and behavioral health providers; Addresses significant social service needs such as food insecurity, housing, special needs services, substance use, transportation, educational and vocational issues.
Arranges, coordinates, monitors, evaluates and advocates for a package of services to meet the specific client’s complex needs..
Requirements: EDUCATION, TRAINING AND EXPERIENCE BSW or BA in related human services field Excellent written and verbal communication skills Bilingual Spanish or Armenian preferred Knowledge of community resources and government benefits Strong advocacy skills Experience working in a health care environment preferred Culturally sensitive and able to work with a diverse population Ability to work in a non judgmental manner and foster positive growth Able to work as part of a team and maintain professional boundariesKNOWLEDGE, SKILLS AND ABILITIES Ability to find solutions when barriers are identified.
Strong documentation skills.
Ability to multi-task and prioritize when needed.

Proficient knowledge of Microsoft Outlook.
Able to travel and attend professional meetings, conferences, trainings and clinic sites.
Demonstrate flexibility, enthusiasm, and willingness to cooperate while working with others in multi-disciplinary teams.
Performs other related duties as assigned.PHYSICAL DEMANDSPosition requires prolonged sitting at a computer, some bending, lifting, stooping and stretching.
Good eye-hand coordination and manual dexterity sufficient to operate a computer keyboard, photocopy machine, telephone, and other office equipment is also required.
