Job Location: Los Angeles and Orange County Remote (1 day in office) Reports to (title): CHW Supervisor Department: Workforce Development Supervises: N/A Working title: Community Health Outreach Worker Classification: Non-exempt Salary Range: $22 hourly Status: Short-Term Hours/Week: 40 hours Effective Date: August 2022 Type of Position: Full-time, 12 month grant funded Revised Date: August 2022 POSITION SUMMARY The goal of the UAII Community Health Outreach Worker (CHW) Outreach Initiative is to address disparities in access for American Indians and Alaska Natives (AI/ANs) to COVID-19 response services, including vaccinations, testing, contact tracing, and PPE, as well as promote health literacy for preventive measures and early detection of disease. As members of our integrated health team, UAII CHW will work to improve health outcomes and address the factors that contribute to the social determinants of health which put AI/ANs at a disproportionate risk for contracting COVID-19. You will ensure that messaging regarding Coronavirus, COVID-19, and COVID-19 vaccination is delivered in an efficient, effective, and culturally appropriate manner. The UAII CHW will also educate the public on UAII s integrated health and human services, recruiting eligible individuals and assisting in the registration process when applicable. UAII Community Health Outreach Workers will also work with AI/AN community members using a case-management approach, assisting community members in 1-on-1 appointments to find and utilize needed health and human services. UAII CHW will be crucial in promoting linkages, establishing referrals to partner programs and services when applicable, and following up to ensure clients have received all necessary resources. ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES Amplify approved COVID-19 messaging and provide general COVID-19 educational outreach to the Native American Community Travel throughout Los Angeles and Orange County Conduct in-person, social media, phone call, and email outreach (according to COVID-19 safety regulations) to provide impacted communities with COVID-19 information about testing, contact tracing, quarantine/isolation, vaccinations, and referrals for other resources Deliver culturally appropriate case management for follow up with eligible participants Provide accurate information regarding COVID-19 vaccines and vaccine safety (what it is, understanding rollout and timeline, description of the different vaccines, how to register when you re eligible, actively registering eligible individuals in high need communities, accessing vaccine distribution sites, review of potential vaccine reactions, and aftercare guidance if a reaction occurs) Promote UAII services and recruit eligible Native Americans living in Los Angeles and Orange County to enroll in UAII services Provide UAII clients and Native American community with linkages to care, support services, and connections to other health and human services when needed Conduct at-home and clinical visits with community members to ensure adequate access to health resources and services Distribute PPE to the Native American community in Los Angeles and Orange County Utilize cloud-based data entry systems to accurately document, track, and measure CHW outreach efforts include learning, navigating, and using new applications/software for phones (Android/iPhone) and computers (Microsoft) Comply with Center for Disease Control (CDC) and UAII training regarding confidential information related to personal information Accept responsibility for other duties, as assigned MINIMUM QUALIFICATIONS Experience & Qualifications: Must be Fully Vaccinated against COVID-19 Case Management Experience (1 year) Outreach or Marketing Experience (1 year) Data Entry Experience (1 year) Access to private transportation Ability to schedule various meetings to discuss the Outreach objectives Ability to explain complex health concepts both 1-1 and in groups Ability to research and provide referrals to health and human services based on client s needs The ability to travel throughout Los Angeles and Orange County for outreach, meetings, and supply pick up Customer Service Skills Strong Communication Skills Time Management Skills Quick Learner with the ability to work remotely when necessary Able to work independently and as a team member who consistently demonstrates professionalism, courtesy, efficiency, excellent internal and external customer service, high ethical standards, and behavior Prior knowledgeable to the needs of the American Indian community/culture Computer Literacy: Must know how to use a computer, mobile phones (Android or Apple) Microsoft Office, and Google Suite Required Education: High School Diploma Required, Bachelor s Degree preferred Certifications: N/A Confidentiality: Maintain strict confidentiality of client and personnel information adhering to HIPAA, professional codes of conduct, State of CA regulations, Privacy Act, and UAII policies and procedures This position has access to sensitive information, and a breach in confidentiality will be grounds for disciplinary action up to and including immediate termination Communication/ Interpersonal Skills: Communicate and deal effectively with people in a professional and courteous manner verbally and in writing Proficient in English Technology Skills: Working knowledge of Microsoft Word, Excel, and PowerPoint Working knowledge of Google (Mail, Calendar, Drive) Other Skills: Available and willing to travel as needed by funding sources Available to work evening and weekend hours as needed Willingness to learn new skills and participate in training sessions relevant to the position POSITION PREFERENCES Experience working with the American Indian/Alaskan Native community Experience working with nonprofit organizations, community groups, and performance-based government contracts Previous experience working in community outreach or recruitment Previous experience working in community outreach or recruitment Knowledge of Indian Self-Determination and Education Assistance Act (Public Law 93-638) WORK ENVIRONMENT Work environment: The work environment characteristics described here represent those employee encounters while performing the primary functions of this job.
