The Hope Street Family Center (HSFC) was established in 1992 as a community program of California Hospital Medical Center. Original program funding was provided by a research and demonstration grant from the federal Head Start Bureau as a national effort to develop more effective ways to support young children and families.

Responsibilities
Position Summary:
The Clinical Services Health Coordinator is responsible for developing and implementing health-related services for pregnant women, children 0-3 years of age, and their families.
Responsibilities include:
- Provide healthcare coordination for children and pregnant women; advocate for the healthcare needs of children and families; provide consultation for home visitation and interdisciplinary team regarding healthcare resources and community services. Serves as liaison to community agencies.
- Plan, develop, and facilitate ongoing training for all clinical staff in the areas of child health and development, pregnancy and maternal health, communicable diseases, general health and safety, environmental health, and nutrition.

Qualifications
Requirements:
- One year of community health nursing, with pediatric or health education background preferred.
- Blingual English/Spanish language skills preferred.
- Bachelor Degree in Nursing.
- Possession of a valid California driver’s license and ability to make home visits to various Los Angeles area locations.
Skills:
- Ability to function as a member of an interdisciplinary team.
- Ability to effectively plan and conduct health education services.
- Demonstrated skill in providing community health services within low-income communities to families from diverse cultural and ethnic backgrounds.
- Skill in teaching and mentoring professionals from a various disciplines and backgrounds.
- Knowledge of the health care system and community health care resources.
- Knowledge of community health theories and best practices.
- Knowledge of the field of family violence and familiarity with reporting laws.
- Demonstrated ability to execute projects in an independent and responsive manner.
- Experience with staff supervision and program development.
- Skill in organizing and maintaining a comprehensive record-keeping system.
- Ability to speak clearly, concisely, and in an organized manner.
- Skill in writing clearly to articulate concepts and ideas in a logical fashion.
- Possession of a valid California driver’s license and ability to make home visits to various Los Angeles area locations.
