The Clinical Social Worker for the Homeless Prevention Program is responsible for providing assessments, treatment plans, and interventions for both intensive case management and mental health services. Services are provided to older adults who are at risk of eviction and homelessness or who are recently homeless due to mental health issues including hoarding disorder. The Clinical Social Worker will work within a team structure along with a social work assistant who will provide care navigation, case management interventions and support to clients. Field based services are provided within the client home setting with the goal of retaining housing, decreasing symptoms of mental illness and hoarding behaviors and to ensure healthy and stable living conditions.
Qualifications:
- Master’s Degree in social work or a related field.
- Registered with the California Board of Behavioral Sciences or the Board of Psychology, Licensed preferred.
- Current unrestricted CA Driver’s License and proof of auto insurance.
- Strong clinical judgement and prior experience required in providing clinical psychosocial assessments, diagnosis, treatment planning and interventions for mental health issues.
- Prior experience required in providing targeted case management assessments, care plans and interventions.
- Care management and community-based service delivery experience required.
- Knowledge of community resources and the aging process.
- Bilingual in one of the following languages: Farsi, Russian or Spanish is a plus.
- Experience conducting field based in-home visits and treatment strongly preferred.
- Excellent communication skills both verbal and written and ability to utilize telehealth platforms.
- Must be computer literate in MS Office applications.
- Culturally sensitive and able to work with a multicultural client/customer population; ability to work in a non-judgmental manner and foster positive outcomes; able to work as part of a team and maintain professional boundaries.
- Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment.
Responsibilities/Essential Functions:
- Provide in-home clinical psychosocial assessments, diagnosis, treatment planning and interventions for mental health issues experienced by older adult clients with hoarding disorder.
- Provide interventions including such activities as: individual therapy; crisis intervention; psychoeducation; motivational interviewing; skills building; group therapy; substance abuse counseling.
- Provide in-home targeted case management assessments, develop care plans, coordinate the delivery of interventions including: declutter and clean out services; moving and storage; legal assistance for eviction resolution; use of harm reduction approaches; linkage with community resources; identifying permanent housing options.
- Assist with outreach in the community for new client referrals.
- Identify and coordinate family and other informal support systems for the client.
- Refer clients to specialist care as needed toward improvement of the client’s functional level.
- Provide timely documentation in keeping with funding requirements and electronic health records.
- May be required to be a Field Work Supervisor and/ or preceptor for Social Work students.
- Participate in multi-disciplinary team meetings as scheduled.
- Participate in weekly individual supervisory meetings.
- Other related duties as assigned.
Supervised hours towards licensure may be available but not guaranteed.
