Scope The Clinical Program Coordinator will be responsible for overseeing all clinical operations for Infectious Disease to include physician workflow development and implementation and performance metric collection and analysis.
In addition, this individual will serve as point of contact for clinical practice issues for all AP physicians and partner hospital sites.
Description of Duties/Essential Functions: (individual duties, assignments, and responsibilities required of the position) Attend meetings and stay up to date on all business initiatives related to service line including: Internal team meetings Cross-departmental internal meetings to develop initiatives Implementation meetings Account management meetings Maintain a robust working rapport with individual providers covering the service as needed by assessing workload, address clinical workflow or practice issues, attend to tech issues that arise (onsite EMR, UL), manage scheduling glitches and communicate changes to medical director, pod and hospital(s) affected Measure, analyzes, generates and presents reports on healthcare processes and patient outcomes to identify and prioritize areas for improvement Participate in all program launches and support with the development of clinical operational aspects of each program Coordinate performance improvement activities focused on specific patient service lines or organizational quality initiatives through the use of specific benchmarks and evidence-based practices Participates in efforts to establish and maintain organizational readiness to meet regulatory requirements based on service line specialty Lead Infectious Disease Committee meetings Ensure appropriate resources are available to support patient care and procedures Collaborate closely with Service Line Chief(s) to improve operations and introduce to the team Collaborate closely with VP of Clinical Program Development to work on team initiatives, develop and report KPIs, identify opportunities within programs to improve communication, efficiencies and processes Supervisory Responsibilities: (personnel supervision, budgets, performance, etc.)

· Bachelor of Science in Nursing from an accredited school of nursing is preferred
· At least two years of experience in Infectious Disease area of care (preferred)
· Knowledge of medical terminology Knowledge, Skills, and Abilities: (technical, interpersonal, etc.)
· Ability to navigate multiple EMR systems
· Excellent computer skills and familiarity with Microsoft Office programs including Excel for data manipulation
· Superior interpersonal communication skills and the ability to exercise empathy when working with patients and their families
· Strong organizational and time management skills
· Demonstrate an understanding of standard clinical procedures, laws and regulations
· Thorough knowledge of medical terminology
· Ability to work independently, but function as part of a team Work Environment: (travel, physical demands, and conditions)

· Prolonged periods of sitting at a desk and working on a computer.
· Must be able to lift up to 15 pounds at times.
