What will you be doing in this role?

Functioning under the Associate Director of Community Health Improvement, the Clinical Program Coordinator, RN, provides necessary education and information on available options, the informed decision-making process, and realistic goal setting in order to empower community members to actively participate in their plan of care; establishes and coordinates a supportive care plan with goals and interventions based upon treatment and individual needs, preferences, beliefs and values; reviews, evaluates and revises the plan of care for appropriateness on an ongoing and timely basis.

Participates in the development of guidelines, protocols, pathways, and other performance improvement activities:
Coordinate community health improvement activities including vaccine clinics, health fairs, health screenings, and other health education outreach events; trains and oversees CS staff and volunteers at events Facilitate outcomes focused, evidence-based programs in identified areas of community need Collaborate with Cedars-Sinai departments such as Nursing, Cancer Center, and Direct Community Benefit programs to implement community health improvement activities Stay current on the latest clinical guidelines and recommendations for chronic conditions such as but not limited to hypertension, heart disease, diabetes, cancer and others Provide high quality health education and prevention, and refer participants to local resources as appropriate Support the development of an overall community health improvement evaluation plan in alignment with the overall Direct Community Benefit impact statement Collect and enter evaluation data according to program evaluation plan; analyze data and provide recommendations to improve programs Provide support to the internal Cedars-Sinai Community Health Advisory Council
Requirements:
Bachelors of Science, Nursing required Registered Nurse in California Valid California Driver’s License required.

Three years’ experience in inpatient or acute care setting required.

What We’re looking for:
Two years’ experience in community health outreach preferred.

Master’s degree in Nursing, Public Health or related field preferred Participation in Cedars-Sinai community health outreach a plus.
