Responsibilities
Under the direction of the Administrative Nurse and Clinical Nurse Manager of the State funded prostate cancer treatment program, IMPACT, you will coordinate all patient care activities.
You will be responsible for the coordination of and data management of clinical activities.
Other responsibilities include:
Coordinating/facilitating patient care and follow-up with contracted providers; Ensuring that program protocols have been followed and data obtained accurately and in a timely manner; Conduct patient recruitment and enrollment; Conduct patient interviews, chart documentation; input data collection and compose case reports; Adhere to program guidelines to ensure quality assurance; disseminate prostate cancer treatment information and education; Prepare weekly summary reports including outstanding provider and treatment issues ensure outcomes meet program policies; Conduct on-line (internet) research addressing community resources for populations-at-risk Prepare written summaries, daily and weekly, for NCMs on patient contact, follow-up and progress; Ensure patient’s receive clinical care within program parameters and as indicated by physician and NCMs; Serve as a clinical translator
