Second Chance Act
Second Chance Act Community-Based Re-Entry Program is a comprehensive, case management-based program that addresses criminogenic risks and needs in a manner that is consistent with participants’ learning styles and abilities to ensure a safe and successful reentry for 170 medium
– to high-risk ex-offenders transitioning from Los Angeles County Jails to downtown and South Los Angeles.
VOALA will partner with LA Sheriff’s Department and LA County Probation Department to implement services.
Pre-release services will include case management, housing planning, job-readiness, and mentoring.
Post-release services will include case management, housing placement, mentoring, and linkages to support services (employment, healthcare, mental health care, substance abuse treatment, education, life skills, and civil legal assistance).
JOB SUMMARY AND PURPOSE
The Clinical Case Management Team provides clients with the highest levels of acuity and co-morbidities individualized care through access to service linkages, counseling, treatment planning, and various crisis intervention methods in order to prepare individuals for further success.
The Lead Clinical Case Manager supervises the services provided to clients who have several psychological co-morbidities that must be managed appropriately for a successful re-entry into society.
As the Lead Clinical Case Manager, this position is responsible for managing most complex client cases.
Additionally, coaches and mentors junior staff.
The position requires a combination of administrative/office duties and field/outreach work.
DUTIES AND RESPONSIBILITIES
Assigns and monitors case load of team: Review daily referrals and screenings of potential clients.
Designates clients to case manager for intake and ensures all supporting documents are verified Conducts comprehensive screenings and assessments with clients to collect functional, environmental, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan Develops support systems to meet client needs, coordinates and facilitates move-in activities, rental negotiations, and service referrals as required Makes home and field visits to assigned clients weekly Monitors case managers on a regular basis to determine quality and effectiveness of services provided Review cases with Program Coordinator/Manager and Clinical Case Management Team Audit case file on a regular basis for completeness and accuracy, and compliance with regulatory requirements (HIPPA, local law enforcement requirements) Conducts crisis intervention as necessary Assists case managers by identifying and coordinating a variety of available services necessary for clients and families to maintain/obtain self-sufficiency and family stabilization Makes referrals to available community providers for therapeutic, social, educational, and medical resources, as appropriate Provides support to case managers, clients and families through ongoing case conferencing, counseling, and advocacy Provides training, supervision, and problem-solving guidance for Case Management Team in close consultation with Clinicians Develops, performs, and assists with workshops pertaining to program services that meet client needs Attends Court as the client liaison as part of case management.
Participates and assists with outreach events Other duties as assigned
Qualifications
REQUIREMENTS:
Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
EDUCATION:
Master’s degree in social work, psychology, or counseling from fully-accredited college or university.
Registered with the Board of Behavioral Sciences as an Associate Clinical Social Worker (ASW) or Associate Marriage and Family Therapist (AMFT)
EXPERIENCE:
Minimum of 3 years of complex clinical experience in program(s) serving homelessness, addictions, under-served populations or prison re-entry/diversion Minimum of 1500 supervised hours working towards Licensure (LCSW or LMFT)
.KNOWLEDGE:
Strong knowledge of case management ethics, HIPAA, Programmatic standards of operation and clinical engagement, clinical documentation methods, client focused ideology.
Knowledge of psychosocial treatment modalities, especially cognitive behavioral therapy (CBT), dialectical behavior therapy skills training (DBT), motivational interviewing (MI), relapse prevention (RP), Seeking Safety Strong knowledge of and ability to work with issues related to homelessness, aging in place, substance abuse, mental health diagnosis, and history of trauma
TECHNOLOGY SKILLS:
Strong Microsoft operating system navigation, including Outlook, Office, Word, Excel, PowerPoint; familiar with Database entry and use.
SKILLS AND ABILITIES:
Mastery of case management skills in social services delivery.
Able to organize and schedule team’s caseload, support efforts of junior case managers.
Demonstrated ability to perform under circumstances of possible emotional stress and conflict, including dealing with un-cooperative clients.
Strong Communication Skills in oral and written English language.
Ability to coach and mentor staff.
Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA, and other federal, state, and local standards, with or without reasonable accommodation, including meeting qualitative and quantitative productivity standards.
Ability to maintain regular, punctual attendance consistent with ADA, FMLA, and other federal, state, and local standards.
