A NON-PROFIT ORGANIZATION PROVIDING MENTAL HEALTH SOLUTIONS

Provide onsite field based mental health, case management, crisis intervention and support services to AB 109/Community Integration Program clients. Work with community resources for linkage and referral including DMH HUB liaison. Be part of a multi-disciplinary treatment team and assess and stabilize according to mental health and basic needs of client. Work with client in treatment planning including CCCP/Client Care Coordination Plan, benefit assistance, and housing stability as needed. Provide direct and indirect client services.
Participate in assertive assessment, evaluation and referral of AB109 clients. Assess client’s level psychosocial impairment, current substance abuse and history, self-care potential, family and support network availability, independent living needs, financial status, education, employment or training needs, and social/recreational needs.
Provide crisis intervention – intervene with the client/others at the onset and/or during a crisis to provide support and assistance in problem resolution and to coordinate or arrange for the provision of other needed services. Liaison with the County of Los Angeles Probation Department HUBS to engage client at referral intake.
Transport clients for needed resources including, but not limited to, Social Security offices, food banks, or appointments as needed in accordance with Center Safety guidelines. Enroll client in the application process for transportation needs including Citi-ride, MTA, or assist in obtaining bus cards, etc. for greater self-sufficiency.

Job Requirements:

Education:
The ideal candidate must have a Bachelor’s Degree in the field of Human Services, or related degree in Social Services., i.e., Social Work preferred. A minimum of four years post-graduation experience in working with forensic population or related field preferred.
Experience:
A minimum of four years post experience in mental health setting with mentally disabled and co-occurring disordered individuals is essential to job function. Knowledge of forensic individuals preferred. Ability to obtain mainstream resources for targeted population and ability to multi-task and work with others required.
Must have a current/valid California driver license, a good driving record and must be insurable by Center’s insurance company.
Bilingual, Spanish preferred.
