The Care Coordinator is responsible for the care of an admission to the skilled care community (SCC) from the initial assessment of the level of care required, to the coordination of the care with the staff, and to the maintenance of the care records.
Ensures Medicare compliance and accurate completion of the Minimum Data Set (MDS).
Serves as the liaison between the community, the payer, and the corporate office.
Coordinates the restorative nursing care program by screening the residents, setting goals for the resident, and updating the goals as needed.
The ideal candidate will meet the following requirements: Current state Registered Nurse (RN) license Minimum of two years’ experience in Minimum Data Set (MDS) and delivery of patient care, preferably in geriatric and/or medical/surgical nursing Current or eligible for certification in Cardiopulmonary Resuscitation (CPR) To protect the health and safety of our residents, employees, and the communities we serve, Acts has implemented a universal COVID-19 and Influenza (flu) vaccination requirement for all employees.
