Maintains schedule of all home health servicesAssists with preparation of nursing call schedule.Receives updated orders from physicians.Takes and prepares intake referrals, and prepares clinical notes for interventions rendered.Verifies Medicare/Medi-Cal benefits.Performs nursing care and other field staff functions as necessary.When requested, provides report to case managers of third party payors after clinical reviewFacilitates services that are ordered by the physician as indicated in the plan of care.Facilitates ongoing interdisciplinary assessment of the patient.Facilitates development and evaluation of the plan of care, under the supervision of the Registered Nurse Clinical Supervisor, and in partnership with the patient, the representative (if any), and the caregiver(s).Acknowledges patient rights including privacy and confidentiality.Consistently displays a professional appearance and conduct in all settings.
Customer service, including interpersonal and inter-agency relationships, is positive and professional at all times.Additional list of duties and responsibilities is provided in the Job DescriptionWhat you have accomplished:Medical AssistantGraduate of accredited nursing program.
