ALL’s WELL: Case Managers (Lvn/Bsw) Needed For Well-Respected Medical Institute In Los Angeles Area

Collaborates with Providers, Managed Care Plans, Housing Navigation Team and community homeless service providers to provide enhanced care coordination for eligible patients and/or entities to ensure a seamless experience for the patient and non-duplication of services2.

Conducts case management services to include intake and assessment, develop comprehensive care plan, implementation of care plan, subspecialty referrals, monitoring and follow-up.3.

Connect Member to other social services/support services and provide appropriate referrals for subspecialty care (i.E.

ophthalmology, behavioral health, podiatry, nutrition, health education, etc.), transportation, food, housing, etc.

Accompany Member to office visits, as needed.4.

Advocate on behalf of Members with health care professionals5.

Use motivational interviewing, trauma informed care, and harm-reduction approaches6.

Coordinate with hospital staff on discharge plan7.

Monitor treatment adherence (including medication)Requirements:1.

AA or bachelor?s degree in Social Work, sociology or related field experience with at least 1-year experience as care coordinator, community health worker or similar responsibilitiesOR2.

High School Diploma or GED with at least 2 years?

experience as care coordinator, housing navigation, community health worker or similar responsibilities.

(Certified Community Health Worker strongly recommended)OR3.

Medical Assistant with at least 1-year experience in case management.AND4.

Experience working with individuals who have multiple chronic conditions, ER visits or frequent in-patients?

admissions or experiencing homelessness.5.

Electronic Health Record experience and/ Data entry skills required.

NEXTGEN EMR is a huge plus6.

Proficient in utilizing Microsoft Word, Excel and Power Point.

6 Good communication and customer service skills.7.

Strong desire to work as part of multi-disciplinary team or care coordinators, LCSW, MSW, PCP?s and MAs.8.
