Hiring is a human process, and they would like to meet you to see if you’re a fit!YouTube Link: https://www.Youtube.Com/embed/4t6XDMAOQHUScreening Questions:Mandatory:Do you have a CA Registered Nurse License?Mandatory:Have you been fully vaccinated for COVID 19?The RN Care Team Manager (RNCM) is responsible for projecting and integrating the Mission and Core Values of the organization in the provision care management services to members of the AHF Medicare and Medi-Cal health plans of California.Key responsibilities include the delivery of care management services as outlined in the Medicare and Medi-Cal contracts, Guidance, All Plan Letters and this Special Needs Plan’s (SNP) Model of Care.
This includes time-sensitive, regulatory compliant assessments with comprehensive diagnosis review and care planning.
RNCMs also engage in Medication Therapy Management (MTMP) during transitions of care that a comprehensive medication review, and medication and adherence teaching and and care plan development that supports retention in care..
The RNCM works in collaboration with the member’s Primary Care Provider and their staff to assist in providing a Health Home.
This includes participation in routine meetings with PCP and staff to share gap lists and non-compliance concerns.
The RNCM may also facilitate scheduling for plan members for preventive care, annual wellness visits and urgent care; collaboration with Utilization Management staff and Medi-Cal Waiver Program staff for In Home Supportive Services (when qualified) to ensure the most comprehensive understanding of member needs.
The RNCMRNCM will also have an understanding of Risk adjustment knowledge and claim review for their assigned members.
The RNCMRNCM will be responsible for the gathering and transmission of information and data to the Care Coordination team and to their assigned primary care providers and staff as appropriate in all care settings, including participation in Interdisciplinary Care Team Rounds with PCPs and other specialists both medical and behavioral.
The RNCM will also participation in activities that increase understanding of the communities served by the AHF and go the extra mile to assure members have access and knowledge of their health plan benefits, community benefits, treatment plan, and medication adherence.The RN Care Team Manager (RNCTM) contributes to AHF’s mission by displaying the qualities of being knowledgeable, caring and dedicated to HIV/AIDS specialty health plan members.
You will lead the care team in:ensuring plan members are engaged in their health care,collaborating with the member and the PCP to establish and develop health care goals and treatment plans,developing personalized care plans which set specific interventions which impact the member’s health & wellbeing,achieving mutually agreed upon goals to maximize the member’s best clinical outcomes,educating members regarding preventative wellness activities and incentives; andacting as the member’s advocate for optimum health outcomes.By engaging in opportunities to increase knowledge regarding community, government, grant funded programs and health plan resources, the RNCTM can ensure the member is optimizing all available health care resources.
You can also assist in improving member focused HEDIS measures, such as preventative wellness, which ensure our members keep healthy or Risk Adjustment which allows us to allocate services to members who are more at risk for poor health outcomes than other members.Enhance the experience of all Managed Care employees when you collaborate with other departments to ensure member needs are addressed.
