LifeStance Health, Inc. seeks a full-time Adult Primary Care Therapist.  This position serves the integrated behavioral health program of a major primary care practice with multiple locations in the Los Angeles metro area.  The candidate would be responsible for providing brief behavioral health assessment, triage, and treatment services to primary care patients, consultation and support to PCPs and other medical staff, and some specialty (ongoing outpatient) mental health services.  In addition to exceptional generalist clinical skill, the ideal candidate will also have a solid foundation in health psychology theory and interventions (e.g. CBT-I, chronic pain management, weight management, diabetes self-management).  Interpersonally, the ideal candidate will be comfortable engaging medical staff in a fast-paced and highly interactive work environment and using flexible problem solving.  The ideal candidate will have the ability and desire to provide services both virtually and in person at primary care practices in the San Fernando Valley, Downtown LA, or San Gabriel areas.  Ability to provide psychotherapy in both English and Spanish strongly preferred.

Is this you?

  • Position requires Licensed Clinical Social Worker (LCSW) or Licensed Professional Clinical Counselor (LPCC) in with active California license
  • Facilitate behavioral health screening program using standardized measures (i.e. PHQ-9, GAD-7, PC-PTSD, AUDIT-C, etc) in an integrated primary care setting. 
  • Participate in identifying patients who would benefit from BH services via outcomes of screening measures and collaboration with PCPs/medical staff.
  • Facilitate smooth transition from medical visit to behavioral health consult for patients.
  • Provide brief, targeted assessments and interventions as appropriate in the primary care setting, and in response to identified referral question.
  • Refer for specialty mental health or other ancillary services as appropriate.
  • Provide bridge care support to patients while facilitating transfer to specialty mental health or other higher levels of care
  • Coordinate treatment with all healthcare team members including, for example: patient, PCP, psychiatric prescriber, specialty medical care providers.
  • In an ongoing fashion, gather and utilize behavioral health symptom and functioning data to inform clinical care and facilitate LifeStance-driven service line and outcome research.
  • Provide curbside consultation to PCPs and other medical staff to coordinate treatment for shared patients and general behavioral health guidance.
  • Communicate with PCPs and other medical providers about diagnosis, treatment plan, and relevant ongoing case information in a timely (usually same-day) and ongoing fashion.
  • Maintain balance of pre-scheduled appointments and open access space in daily schedule in order to accommodate same-day and urgent consult requests from PCPs
  • Provide medical staff education on behavioral health topics as relevant to primary care practice.
  • Remain abreast of ethical and effective coding/billing practices in order to prioritize program financial sustainability as well as patient and medical staff needs.

Licensed Therapists are a critical part of our clinical team. We’re seeking Licensed Therapists that are:

  • Fully licensed and credentialed in California LPCC or LCSW  
  • Experienced in working with Senior populations (55+).
  • Bilingual Spanish/English strongly preferred
