We are licensed Home Health Agency in the State of California and are preparing to undergo our Medicare Certification.
This position may require some field work initially, with the expectation to transition to all in-office/supervisory work as we grow and expand.?Just a little background on us and our company: Desiree and I are both passionate physical therapists who have been working in the home health setting for other companies the past several years.
It has always been our goal to provide the highest quality of care to our patients and we were never fully satisfied working for other agencies, so we decided to create our own company.
It is our hope that by forming better relationships with our staff and colleagues, we can create a better working environment for everyone involved, placing the patients?

A registered nurse with a baccalaureate or higher degree in nursing or other health related field?
3 years of experience within the last 5 years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity?
Demonstrate expertise in clinical decision-making for the patient populations of the home health agency to meet the needs of his or her patients?
Valid nursing license, CPR certification, and reliable care/auto insurance?
Excellent organization and time management skills?
Ability to effectively communicate, both verbally and in writing?
Bi-Lingual or Multi-Lingual preferredJob Duties:?
Provide care and service that is honest, respectful, and patient-centered?
Oversee all nursing personnel (RNs, LVNs, HHAs)?
Work in coordination with Nursing Supervisor to ensure patients?
nursing needs are adequately met and expectations exceeded?
Coordinate admission of patients to the nursing department?
Complete initial patient assessment, to include completion of OASIS documents?
Resolve nursing-related deficiencies when needed, ensuring individual patient needs are continually assessed and met?
Actively collaborate with other departments to ensure highest quality of care?
Willing to be accessible by phone during designated on-call shifts?
Coordination of patient care services and shall be responsible for all activities relevant to the patient care services?
