Responsibilities Screen, monitor, and educate members regarding acute and chronic conditions and promote healthier lifestyle behaviorsAdminister tests for diseases and monitor progress through blood pressure readings, lipid, glucose, A1C, BMI measurements and others.Participate in seasonal vaccination, and other types of testing as needed.Participate in Health Fairs and Outreach Events at New York City and outer borough locationsEducate members (in-person, virtually, and telephonically) regarding acute and chronic conditions and healthier lifestyles, and record clinical data for statistical analysisAdminister vaccines to membersCounsel members on Health Benefits provided by 1199SEIU and refer to appropriate internal departments and community agenciesContact physicians and/or specialists regarding member health statusPerform additional duties and projects as assigned by management QualificationsValid New York State Registered Nurse (RN) required; Bachelor’s degree in Nursing preferredMinimum two (2) years nursing experience in ambulatory care, community health care or medical/surgical requiredMust have strong experience in clinical assessment, patient education, motivational interviewing, critical thinking, and independent decision-makingIntermediate computer and digital literacyBasic skills in Microsoft Outlook, Word and Excel preferred; bilingual in Spanish preferredMust have excellent oral and communication skillsAbility to carry a laptop to different sites.Valid NYS Driver’s license preferred with travel in New York Metropolitan areaAbility to work flexible hours including evenings and weekends as needed
