Responsibilities include: promoting growth and independence with personal needs and ADLs, monitoring health and safety, building advocacy, decision making and community connections, and completion of required documentation.Duties and Responsibilities of a Self Direction Support Staff:Provision of services for participants as outlined in the Self Direction plan.
Tasks may include but not limited to:Bathing/showeringGrooming (shaving, make-up, hair care, etc.)DressingToiletingCare of catheter, bags, etc.Transferring, positioning, liftingExercise, stretching, and sensory activities as neededUtilization of adaptive equipmentMeal prepFeedingHousekeeping including laundryShopping for participant, including grocery, personal items, clothingTransportation as neededExplore community opportunities to enhance relationship building and develop leisure skillsAssist with money management including banking/budgetingAssist with scheduling and participating in activities and appointments as neededAssist with job/volunteer activitiesIncrease/monitor safety skills in the community and homeImplement participant’s goals as written in Self Direction planImplement safeguards and behavior guidelines/plans as writtenSupport participants by providing opportunities to increase decision-making and advocacy skillsCompletes required documentationComplete all program documentation in a timely manner and per Agency standards.Completes incident report forms thoroughly and in a timely manner per Agency standardsFollow procedures to report emergencies or medical/safety needs/changes of participantDocument mileage per agency proceduresCommunity and TeamworkWork collaboratively with participants, families, program staff, co-workersAttend meetings as directed by supervisor/FI (Fiscal intermediary)Attend all required trainings as directed by supervisor and maintain training certificationsParticipate in Agency-wide events, advocacy, and/or community education.Follows Agency standards and proceduresProvides person-centered, quality servicesObtains and demonstrates core competenciesFollows Agency standards and proceduresMaintains compliance with applicable rules and regulationsOther duties as assignedEducation/Experience:High school diploma/GED preferred.
Experience working with people with disabilities preferred.
Basic word processing and e-mail skills are required.Conditions of Employment:Must possess valid New York State driver’s license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs.
Support staff providing respite only that does not include driving tasks may be exempt from the driver license requirement.
Must be able to lift 75 pounds.
Must have an electronic device that supports the Agency’s EVV (electronic visit verification) system and be willing to use the minimal personal data required for EVV.
Successful completion of required orientation and annual training.
Must be able to demonstrate teamwork and professionalism.
Must be able and willing to demonstrate direct care duties, including personal assistance, as need arises.
Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office of People with Developmental Disabilities, Chapter 575 of the Laws of 2004.
Must participate in successful database checks through the HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.Hourly rates are authorized by the Office for Persons with Developmental Disabilities (OPWDD) and approved in participant Self Direction budgets.
