Job Summary :
Perform annual wellness visits and health assessments on a geriatric population with chronic conditions
Provide patient with health education and teaching
Assist in closing quality care gaps (screenings and labs)
Team player attitude is a must and have a willingness to contribute to the continuous enhancement of care delivery workflows
Qualifications:
Must have a active NP, or MD License for CA
National Certification, experience in primary care/adult/geriatric, EMR skills preferred
Compensation:
Position is an Independent Contractor, and it is based on per visit.

Preferred experience:
Previous experience with In-home risk Assessment.
Languages
Any bilingual candidate is a Plus.
