MDS Coordinators (RN Only)

Currently seeking an immediate Full-Time RN MDS Coordinator.

MDS Coordinator Duties:

* Completes the appropriate nursing assessments on the MDS form.

* Coordinates the development and documentation of the individualized resident care plan.

* Completes Medicare PPS and HMO tracking forms and required.

* Signs and dates all pertinent RAI document, including the attestation statement

* Conducts resident observation rounds to ensure appropriate interventions are implemented to promote resident’s optimal functions and well-being.

MDS Qualifications:

* Current & unencumbered California license as a Registered Nurse.

* Minimum 1 year nursing experience, preferred, in a nursing facility or long term care.

* Completion of MDS training course within 6-months of employment or assignment.

* Must possess excellent organizational, communication
– both verbal & written, and computer literacy skills.

Full-Time

Shift: AM

Job Type: Full-time

Schedule:

* Monday to Friday

* Weekends

Experience:

* license nurse: 1 year (Required)

* acute and/or long term care: 1 year (Required)

Education:

* Bachelor’s (Preferred)
