Home Infusion Pharmacist P.I.C.

Job description Objective: Pharmacist In Charge Under moderate direction, coordinates and assists in managing/participating in pharmacy services that include compounding, quality assurance, dispensing, and clinical monitoring activities.

Oversees the clinical services offered by the branch as their related to both infusion and / or specialty services provided.

Educational Qualifications & Experience: * Graduate of an accredited School of Pharmacy with PharmD.

or B.S.* Current license and registration in State of California* Competencies Required for Effective Performance* Minimum 2 year experience in infusion pharmacy.* Without infusion pharmacy experience, please do not apply.* Proven experience with dosing and monitoring therapeutic IV antibiotics and TPN.* Long term experience with CPR+* Proven experience with home infusion therapy, and related diagnoses.* Knowledge of the management of infectious diseases, with emphasis on compounding IV infusions.* Excellent communication skills Principal Tasks: * Participates in decisions regarding the pharmaceutical services within assigned scope including:* Assessing the patient’s general pharmaceutical needs.* Participating in the overall plan of care.* Participating in the overall quality assessment and improvement plan.* Receives physician prescription orders and evaluates the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration, and stability.* Supervises the filling, receiving, compounding, and dispensing activities involved in providing finished product to the patient.* Provides shift supervision to Pharmacy Technicians, Pharmacy Interns, Shipping and Delivery Personnel.* Ensures that, laboratory tests have been ordered and the results are obtained and discussed with the nurse and the physician in a timely manner.* Provides patient counseling, education, and monitoring* Ensures safety of patients and personnel by exercising good pharmacy practice in the process of drug therapy instructions to the patient.* Provides clinical interventions* Assesses responses to therapy, patient compliance, and ongoing review of drug regimens; reports and manages adverse events.* Assists the physician, nurse, and patient with clinical and technical support.* Documents all communication with physicians, nurses, and patients in the patient chart.* Assists in the maintenance and upkeep of all necessary paperwork required by State and Federal agencies as well as the URAC.* Completes all competency/skills assessment requirements.
