Family Pact, and CHDP are just some of the programs the clinic participates in. Candidate must be able to work independently and bilingual English/Spanish is a plus. Candidate would be responsible for sick visits, physicals, annual wellness exams for seniors chronic disease management, and act as a coordinator for primaty care and specialty referrals.
