Schedule: Monday Friday (8:00am 4:30pm)Benefits: Medical, Vision, Dental, Life Insurance, 403(b) Retirement planESSENTIAL DUTIES AND RESPONSIBILITIESUnder the supervision of the Assistant Director and Director of H& W, the Case Manager’s responsibilities are to:Facilitate intake for individuals who are referred into interim housingProvide case management (linkage to income/benefits, supportive services, housing opportunities etc.) and follow-up services to a caseload of 15-25 individualsPerform a comprehensive needs assessment (screening and intake) for all case managed clients, and develop an individualized service plan that is based on client’s needs, barriers and goalsPromote client independence by collaboratively establishing patient care goals, providing information to patients regarding their medical condition and medications, and supporting self-care skills.Create a supportive environment and ensure clients’ progress towards permanent supportive housing and psycho-social rehabilitationUtilize a client-centered approach that includes motivational interviewing, harm reduction, trauma informed care and housing first models Facilitate enrichment and life skills groupsProvide crisis intervention when neededWork directly with TPC nursing staff on collaborative and whole person careWork directly with referring partners on oversight and acceptance of appropriate referralsCoordinate with primary care providers’ medical teams to ensure that clients’ medical needs are met.Coordinate medical transportation to support clients’ needsMaintain a cooperative relationship among care teams by communicating information, responding to requests, building rapport, participating in team problem-solving methodsMaintain accurate documentation of client information (intake, referrals, progress notes and service delivery)Maintain accurate client records and statistical data (manually and via computerized database system)Participate in scheduled weekly clinical and case file meetings, monthly staff meetings, trainings and retreatsDevelop effective, trusting relationships with clients using a client centered approach that includes motivational interviewing, harm reduction, trauma informed care and housing firstUtilizing harm reduction approach to provide health services support to individuals experiencing homelessness with acute and chronic medical conditionsCollaborate with local Federally Qualified Health Centers (FQHCs) and other medical facilities to coordinate medical and mental health care for clientsCommunicate effectively and in a timely manner with management team, peers, collaborators, and clientsResearch available community resources and make referralsCoordinate information sharing among staff; consult as needed;coordinate and disseminate information to guestsFacilitate bi-weekly caseload group and weekly House Meeting as neededCollaborate with other projects/agencies who share case managementQUALIFICATIONSBA or equivalent work experience in social services organizationMinimum of two (2) year of experience in case management preferredCNA/MA background, experience coordinating medical care or experience in a healthcare environment preferredDemonstrated knowledge of and experience in dealing with issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)Skilled in non-violent crisis interventionAble to work in a high tolerance Harm Reduction and Trauma Informed Care model with members who have multiple barriersDetail oriented with strong time management, organizational, written, verbal, and computer skillsAble to learn quickly and work effectively with a wide range of constituenciesStrong interpersonal and team building skillsStrong knowledge of Word Document, Excel and use of Emails systems (Outlook, 365, OneDrive)Knowledge of social services agencies and community resourcesValid CA Driver’s license and an acceptable driving recordAble to obtain and maintain CPR/1st Aid certificationWork EnvironmentCombination of field and office environmentRegularly required to sit, stand, bend;
