RN Patient Flow Coordinator – Transfer Center – Per Diem 12 Hour Rotating (Non-Exempt) (Union)

The Patient Flow Coordinator has primary responsibility to facilitate admissions, transfers from outside facilities, and managing patient flow and bed placement.

The Patient Flow Coordinator interacts with physicians, nursing staff, CAS, PACU, clinic staff, Patient Access, and EVS to effectively communicate patient bed placements to coordinate patient throughput.

The Patient Flow Coordinator has centralized authority for the electronic bed control throughout Keck Medical Center of USC.

The individual will utilize the electronic bed board to ensure timely and appropriate patient placements.

The clinically sound and experienced Patient Flow Coordinator will facilitate the admission/transfers, perform telephonic triaging of calls, refer to appropriate Keck physicians, facilitate conference calls and manage patient flow.

The individual occupying this position must demonstrate appropriate, diplomatic, and sensitive communication and interaction skills, all while working in a fast-paced, acute healthcare environment with multiple distractions and ever-changing priorities.

Minimum Education:
• Specialized/technical training Graduate from an accredited school of Nursing required.

• BSN or Bachelor’s Degree in a related field is strongly preferred.

Minimum Experience/Knowledge:
• 5 years of nursing experience in an acute care setting required.

• 1 year Critical Care, Transfer Center OR similar nursing capacity experience where exercised clinical judgment, triaged cases, facilitated transfers, performed bed assignment and managed patient flow required.

• 3 years of case management experience preferred.

• Demonstrates ability to utilize effective, appropriate, and diplomatic oral and written communication skills.

• Demonstrates ability to utilize critical thinking and problem solving skills to work independently.

• Proficient computer skills.

• Highly organized and detail oriented.

• Patient/customer focused.

• Must have ability to effectively multi-task and perform in a fast-paced environment with multiple distractions and ever-changing priorities.

Required License/Certification:
• Valid California Registered Nursing license.

• Current BLS for Healthcare Provider from American Heart Association from American Heart Association.

• Clinical certification preferred.

• Fire and Safety Certification.
