Compensation: $65,000.00
– $75,000.00 City, State: Req’d Qualifications: LVN or LPN with current California licensure
AMI Network is partnered with the 3rd largest Home Health companies in California that is looking for an LVN Home Health Case Manager their LA office!
As an LVN Case Manager, you will leverage your awesome organizational skills to monitor patient care and planning.

Mon-Fri 8:30a-5:00p schedule with no on-call (work-life balance is a big priority here) This job is 100% case management from their LA Office.

Leads effective development and completion of discharge plans including reviewing discharge barriers and providing patient/family education Monitors plan of care to reflect admission database, on-going findings, age-appropriate care, cultural specific needs, and appropriate acuity Discusses w/ patient, family/significant others about the plan of care Monitors the patient’s progress based on the plan Revises plan on an ongoing basis based on patient condition and evaluation of progress in coordination with the clinical team Answers patient questions about medication purpose, side effects, and administration instructions
LVN Care Manager Qualifications:
Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) with current California licensure At least 1 year of clinical case management experience in a Home Health, Skilled Nursing Facility (SNF), Assisted Living or Nursing Home, Hospice, or other setting that provides long-term care Minimum of 1 year of Case Management / Care Coordination experience
Final salary per employer discretion and commensurate with experience.
